Main duties of the jobRole Requirements - Working independently you will provide a high quality of service to patients and their families, ensuring their care and treatment needs are met. You will need to have ACP experience of working with adults with minor illness, long terms conditions and complex health needs. You will be exceptionally organised with a high-level of attention to detail. You will naturally possess excellent inter-personal skills, and an ability to consult and positively engage with key stakeholders across the organisation.
