Are you a compassionate, motivated and patient-focused General Practice Nurse looking to make a real difference within primary care? We are seeking an enthusiastic nurse to join our supportive and progressive practice, caring for approximately 8,500 patients across three sites in North Halifax.

The successful candidate will deliver a broad range of nursing services including chronic disease management, cervical screening, immunisations and vaccinations, wound care, health promotion and preventative care. You will work closely with GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Healthcare Assistants and the wider Primary Care Network to provide safe, effective and compassionate care.

Main Duties of the Job
The successful candidate will provide high-quality nursing care to patients within a busy and supportive general practice environment. Working as part of a multidisciplinary team, you will play a key role in promoting health, preventing disease and supporting patients to manage long-term conditions.

Your responsibilities will include delivering routine and preventative nursing services, undertaking clinical assessments, providing evidence-based advice and education, and contributing to the ongoing development of safe, effective patient care. You will work collaboratively with GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Healthcare Assistants and other healthcare professionals to ensure patients receive coordinated, person-centred care.

Job Responsibilities
Experienced Practice Nurse General Practice Essential
Full Time | Permanent | Competitive Salary (Dependent on Experience) | Supportive Leadership Team

Essential Requirement
Applicants must have previous experience working as a Practice Nurse within a UK GP Practice. Applications from nurses without General Practice experience will not be considered.

Applicants must also have experience in:
  • Chronic Disease Management
  • Cervical Cytology (Smears)
  • Childhood and Adult Immunisations
  • Wound Care
  • Venepuncture

Main Duties and Responsibilities
The successful candidate will:
  • Deliver safe, effective and evidence-based nursing care.
  • Manage and deliver chronic disease clinics in line with current guidelines and QOF requirements.
  • Undertake cervical cytology screening.
  • Administer childhood, adult and travel immunisations and vaccinations.
  • Provide wound care, dressings and removal of sutures.
  • Perform venepuncture and routine health assessments.
  • Deliver health promotion and lifestyle advice.
  • Support minor surgery and specialist clinics where required.
  • Make appropriate referrals to primary, community and secondary care services.
  • Maintain accurate and contemporaneous patient records.
  • Participate in clinical audit, infection prevention and quality improvement activities.
  • Contribute towards achieving practice, PCN and contractual objectives.
  • Work collaboratively with the wider multidisciplinary team to ensure excellent patient outcomes.

Professional Responsibilities
  • Maintain current NMC registration and work within the NMC Code of Conduct.
  • Participate in clinical supervision, training and continuing professional development.
  • Support and contribute to clinical governance and safeguarding processes.
  • Promote evidence-based practice and continuous service improvement.
  • Participate in team meetings, audits and practice development initiatives.
  • Assist with the review and implementation of policies, protocols and procedures where appropriate.

Person Specification
Essential - Experience
  • Experience working as a Registered Nurse.
  • Previous experience working in General Practice or Primary Care.
  • Experience of delivering evidence-based patient care.
  • Experience undertaking clinical assessments and health promotion.
  • Experience administering immunisations and vaccinations.
  • Experience in wound care and management.
  • Experience working effectively within a multidisciplinary team.
  • Excellent communication and interpersonal skills.
  • Ability to prioritise workload and work independently.
  • Good IT skills and accurate clinical record keeping.

Desirable - Experience
  • Experience managing patients with long-term conditions.
  • Experience undertaking cervical screening.
  • Experience using clinical systems such as SystmOne.
  • Experience participating in quality improvement or clinical audit.

Essential - Additional
  • Compassionate, professional and patient-centred approach.
  • Commitment to delivering high-quality, safe clinical care.
  • Excellent organisational and time management skills.
  • Understanding of the Quality and Outcomes Framework (QOF).
  • Knowledge of infection prevention and control in primary care.

  • Willingness to undertake further training and professional development.
  • Flexible approach to working across the practice's three sites when required.

Desirable - Additional
  • Understanding of current NHS and Primary Care priorities.
  • Experience supporting healthcare assistants or student nurses.
  • Full UK driving licence and access to transport (if required for cross-site working).

Essential - Qualifications
  • Registered Nurse (Adult) with current NMC registration.
  • Evidence of continuing professional development (CPD).
  • Competent in immunisations and vaccinations (or willingness to undertake training).
  • General Practice Nursing qualification.
  • Cervical cytology (smear taker) qualification or willingness to complete training.
  • Independent prescribing qualification or willingness to undertake training (if applicable to role).

Desirable - Qualifications
  • Long-term condition management qualifications (e.g. Diabetes, COPD, Asthma).
  • Mentorship/Supervision qualification.
  • Basic or Immediate Life Support certification (up to date).
