Job summaryKings Medical Practice has a great opportunity for a Long Term Condition Practice Nurse to join our forward thinking training practice, in Normanton, just off the M62 corridor.
Our large modern building was personally designed with the intention of hosting all services at one site, for the benefit of patients and our staff and includes 24 fully air conditioned consulting rooms, an operating theatre, education suite along with ultrasound, dermatology, neurosurgery, podiatry, community nursing and an NHS dentist.

Our extensive pharmacy team lead on all medication activity including acute and repeat prescribing, reconciliation of medication, SMRs, promoting eRD and all quality initiatives.
We are committed to freeing up clinicians to focus on clinical work supported by our diverse highly trained multidisciplinary team.
We have a dedicated multidisciplinary team of ANPs, PAs, and Specialists nurses supporting patient care.

Main duties of the job- 16 hours per week

Job responsibilitiesPurposeTo work as part of a multidisciplinary team to enable service delivery and management of high quality evidence based care to meet the health and social care needs of the practice population.
Job Summary- Assess, plan, develop, implement and evaluate treatment programmes that promote health and well being
- Assess, plan, implement and evaluate individual treatment plans for patients with a known long term condition
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
- Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
- Work with patients in order to support adherence to prescribed treatments
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
- Recognise, assess and refer patients presenting with mental health problems
- Implement vaccination and immunisation programmes
