Job summary
Green Valleys Health is looking for a friendly, well organised and dedicated Pharmacy Technician to become an integral member of the general practice team, working within their professional boundaries in practices within the Primary Care Network (PCN). This role will involve working under the supervision of the GP prescribing lead and alongside the existing Pharmacy Technician and Prescription Clerks within the prescription team to ensure safe, accurate and timely supply of prescribed medicines to patients. The Pharmacy Technician will provide technical and administrative support to the GP prescribing lead, other clinicians, and the wider practice staff. They will support the PCN and practice clinical teams with queries around medication from patients. They will be an advocate for medication safety, reduced medicines wastage and medication optimisation across the PCN and practices.

Main duties of the job
  • Assist the Prescription Clerks with medicines reconciliation to ensure accurate transfer of medications across health settings when receiving discharge summaries and clinic letters from secondary care.
  • Conduct medicines optimisation tasks including effective medicines counselling, supporting medication reviews and medicines reconciliation.
  • Maintain strong working connections with local pharmacies to ensure continued patient safety between settings and prompt query resolution.
  • Provide specialist expertise, where competent, to address both the public health and social care needs of patients including lifestyle advice, service information, and help in tackling local health inequalities.
  • Maintain a central role in the clinical aspects of shared care protocols and local formulary and liaise with hospital specialists for more complex patients.
  • Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
  • Undertake medication safety tasks such as high-risk medication monitoring and eclipse radar safety alerts.
  • Provide support and guidance to the prescription clerks through supervision and query resolution.
  • Work collaboratively with the existing Pharmacy Technician to develop prescription team related policies and procedures and maximise patient outcomes.

Job responsibilities
  • Assist the Prescription Clerks with medicines reconciliation to ensure accurate transfer of medications across health settings when receiving discharge summaries and clinic letters from secondary care.
  • Conduct medicines optimisation tasks including effective medicines counselling, supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
  • Maintain strong working connections with local pharmacies to ensure continued patient safety between settings and prompt query resolution.
  • Provide specialist expertise, where competent, to address both the public health and social care needs of patients including lifestyle advice, service information, and help in tackling local health inequalities.
  • Maintain a central role in the clinical aspects of shared care protocols and local formulary and liaise with hospital specialists for more complex patients.
  • Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.
  • Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits, supporting quality improvement measures and contributing to the Quality and Outcomes Framework (QOF) and enhanced services.
  • Undertake medication safety tasks such as high-risk medication monitoring and eclipse radar safety alerts.
  • Provide support and guidance to the prescription clerks through supervision and query resolution.
  • Work collaboratively with the existing Pharmacy Technician to develop prescription team related policies and procedures and maximise patient outcomes.
  • Attend a formal appraisal with your manager at least every 12 months. Once a performance/training objective has been set; progress will be reviewed on a regular basis so that new objectives can be agreed.

There is potential scope for the Pharmacy Technician to engage in Pharmacy Technician-led medicines optimisation clinics following appropriate training and under the guidance of the prescribing lead GP.

Qualifications
Essential
  • Registered with the General Pharmaceutical Council (GPhC)*
  • Level 3 Pharmacy Technician qualification or above*
  • *We are open to accepting students that are due to register as a Pharmacy Technician within the next 3 months if the role is right for them.

Desirable
  • ACT accredited
  • Medicines optimisation
  • Completed Primary Care Pharmacy Education Pathway (PCPEP) or equivalent

Experience
Essential
  • Good IT skills
  • Excellent communication skills
  • Ability to work independently and collaboratively as part of a team
  • Willingness to learn and advance within the role
  • Good organisational skills
  • Able to work within own professional and clinical competencies
  • Evidence of CPD completion

Desirable
  • Experience working in primary care and community pharmacy and/or secondary care
  • Understanding and knowledge of healthcare provision in GP practices, QOF and enhanced services
  • Good clinical system IT knowledge of EMIS/SystmOne/Vision/Docman/Eclipse
  • Good knowledge of local formulary and guidelines (BNSSG), as well as national guidance (NICE, BNF etc)
  • An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners
