Job summary
Dementia UK and IMP PCN are working in collaboration to deliver an Admiral Nursing service across the IMP PCN network. The Admiral Nurse will be part of the PCN integrated and multidisciplinary team and will have a dual role in delivering specialist dementia care and support for people living with dementia and their families when times are difficult, challenging and complex by providing expert guidance and also by supporting best practice of other professionals, making the best use of available resources.

Main duties of the job
The Admiral Nurse will lead, deliver and coordinate relationship-centred dementia care within operational areas of IMP PCN, in order to improve the experience of carers/families affected by dementia with complex needs.

About us
IMP PCN is a collaborative network of nine GP practices serving North Lincoln and the surrounding villages, providing joined-up primary care services to a population of approximately 75,000 patients. The network brings practices together to deliver integrated, proactive care closer to home while improving access, population health outcomes and the sustainability of general practice services. Members practices include, Abbey Medical Practice, Cliff House Medical Practice, Glebe Park Surgery, Lindum Medical Practice, Minster Medical Practice, The Ingham Surgery, Nettleham Medical Practice, Welton Family Health Centre and Willingham by Stow surgery.

Job responsibilities
The post holder will
  • Be an experienced RGN/RMN with considerable knowledge of dementia care. NMC registration required.
  • Perform biopsychosocial specialist nursing assessments to support a large caseload of families affected by dementia with complex needs from peri diagnosis to the end of life
  • Facilitate improvements in end of life care for people with dementia through the development of advance care plans
  • Provide inspirational professional leadership and direction for the delivery of high-quality dementia care
  • Establish excellent and seamless pathways of care for patients living with dementia and their carers
  • Apply the Admiral Nurse Competency framework at an enhanced specialist level including person-centred care, therapeutic skills, and triadic relationships, sharing knowledge, best practice and critical reflective practice
  • Promote the physical, social and psychological health and well-being of carers and people with dementia, including understanding the interaction between dementia and other co-morbidities
  • Supporting carers and families through the complexity surrounding loss, transitions and changing relationships
  • Act as an expert and collaborative advanced practitioner who will provide advice, education and support for people with dementia, families and members of the multidisciplinary healthcare team
  • Support the delivery of key quality and performance indicators agreed by Dementia UK and IMP Primary Care Network

Person Specification
Qualifications
Essential
  • Registered RGN or RMN
  • Postgraduate qualification in relevant field, or evidence of specialist training/education within the field of dementia equivalent to this level
  • Evidence of continued professional development/training in relevant areas, including dementia and leadership
  • Commitment to continued professional and practice development

Desirable
  • Further specialist training/education within the field of dementia equivalent to a Masters level

Experience
Essential
  • Significant post registration clinical experience of supporting people with dementia and their carers/families
  • Experience of: completing holistic nursing assessments, providing bio-psychosocial interventions, working as an autonomous practitioner, professional supervision and development/mentoring of staff, audit/evaluation of clinical practice, collaborative/multi-agency working
  • Experience in service specific setting
  • Experience of facilitating and delivering training/ development of staff

Desirable
  • Experience in service specific setting

  • Experience of supporting practice development/ quality improvement
  • Experience of service monitoring, reporting and evaluation
