We are recruiting for an Interim Transitional Care Lead who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

This is an exciting opportunity to lead and shape the Transitional Care service, ensuring the delivery of safe, compassionate, and high-quality care for babies and their families.

As the Transitional Care Lead, you will provide visible leadership to the multidisciplinary team, drive service improvement through ATAIN reviews and quality initiatives, and play a key role in enhancing patient outcomes, staff collaboration, and the overall patient experience.

Main duties of the job

As the Transitional Care Lead, you will provide day-to-day leadership of the Transitional Care service, ensuring the delivery of safe, effective, and family-centred care.

You will work closely with the multidisciplinary team to coordinate patient care, support effective communication, and maintain high clinical standards.

The role includes leading ATAIN reviews, driving quality improvement initiatives, implementing learning from reviews, and ensuring compliance with governance requirements.

You will also act as a visible clinical leader, supporting staff development, fostering a positive team culture, and working collaboratively with maternity, neonatal, and wider Trust teams to continually improve outcomes and experiences for babies and their families.

Qualifications / Training

Essential criteria
  • RN/RM/RSCN with neonatal intensive care experience and neonatal professional qualification.
  • Evidence of ongoing professional development
  • Newborn Life Support Course
Desirable criteria
  • Degree qualification or working towards it

Previous Experience

Essential criteria
  • Experience of intensive and high dependency neonatal nursing reflecting evidence based practice.
Desirable criteria
  • Interest in teaching and supervising of students and junior staff
  • Demonstrate an awareness of change management

Skills

Essential criteria
  • Ability to assess, plan, evaluate neonatal nursing care
  • Competent in the care management of sick infants
  • Ability to manage own case load
  • Ability to plan and prioritise work
  • Good communication and interpersonal skills
Desirable criteria
  • Participation in audits
  • Experiencing in producing policies and guidelines
  • Good IT Skills

Knowledge

Essential criteria
  • An understanding of the philosophies of current issues in neonatal nursing
  • Understanding of the principles of clinical governance
  • Understanding the importance of evidence based practice
  • Ability to care for the sick newborn

Essential criteria
  • Experience and evidence of engagement around equality, diversity and inclusion issues in relation to policy, service development and service delivery in respect of both services to users and the management of staff
