Job summary
The Garth Surgery has an exciting opportunity for an experienced Clinical Pharmacist to join our practice team.

This person will be responsible for supporting the practice in implementing effective and safe medication management. They will identify areas for improvement and support the wider team.

They will be motivated, enjoy a challenge in a fast paced environment but ultimately will find the role rewarding.

Main duties of the job
The successful Pharmacist will provide structured medication review services to patients, either F2F or via telephone.

They will identify areas where patients may be at risk of harm from medicines.

About us
The Garth Surgery in Guisborough is nestled at the edge of the Cleveland hills and maintains access to major urban centres alongside spectacular countryside and the Yorkshire coastline.

Our practice has 7 partners, 2 salaried GP's and 3 ANP's alongside a hardworking nursing and administration team. We are a training practice and active in research. We operate from a recently refurbished and extended practice building. We have a growing list of nearly 11,700 patients and use Systmone clinical software.

Job responsibilities
Clinical Responsibilities
Utilise the Clinical Pharmacist skill set to allow General Practitioner appointments to be reserved for appropriate caseload, reduce waiting times for appointments, increase access to healthcare, improve screening and diagnosis of chronic and common ailments and reduce A&E admissions and attendances

Provide direct care to patients with long-term conditions, minor ailments and other clinical areas within scope of practice

Provide professional input to practice teams working to improve patient outcomes and reduce avoidable admission to hospital

Liaise with providers including pharmacists in the community and hospital settings to ensure accurate and effective medication management

Provide pharmaceutical support to practices such that medication related processes are reviewed, best practice principles applied, and evidence-based values are embedded in all activities

Utilise the Clinical Pharmacist skill set to directly assist patients in the safe, effective use of medicines to improve outcomes and quality of life

Provide professional advice via telephone and secure digital communication.

To support the delivery of quality improvement in pharmacy led clinical domains such as the Impact and Investment Fund, Medicines Management QI within the PCN DES and CQC requirements.

To support practice teams with day-to-day practice such as medicines reconciliation, repeat prescribing practice and support with queries from patients.

Ensure optimum benefit for prescribed medication through high levels of concordance achieved by support for patients and carers.

Support the care of complex patients in their own homes or in nursing or residential accommodation as part of a multidisciplinary team

Utilise the Clinical Pharmacist skill set to ensure NHS resources are used in a cost-effective manner to maximise outcomes whilst reducing wastage

Provide professional leadership on matters relating to medicine optimisation and quality improvement

Day to Day Duties
Apply clinical expertise in Medication Reviews/ Structured Medication Reviews, face-to-face, remotely or within a patient's place of residence, to deliver high-quality patient facing services that are centred on and tailored to each individual patient's needs

Manage and deliver clinics in long term conditions in collaboration with and support from allied healthcare professionals. Deliver clinics in specialist areas appropriate to clinical competence, which may include, minor ailments, frailty, elderly, renal/hepatic impairment, high-risk medications, frequent attenders in secondary care

Prescribe medication within the limits of the individual professional competence

Pro-actively and prospectively reduce inappropriate polypharmacy and over-diagnosis through systematic, evidence-based de-prescribing

Promote the principles of public health; educate and motivate patients to make healthy lifestyle choices and access services designed to assist health improvement e.g., smoking cessation, dietetics, rehabilitation

Discuss patients with complex needs at local MDT meetings and refer to the ICPT in line with recommendations

Provide professional telephone/ email advice and support to patients and their carers

Reconcile the medications of patients whose care is transferred back to primary care in a timely and effective manner, liaising with patients and community pharmacies to ensure patients receive appropriate medication post-discharge

Interface between community, hospital and primary care pharmacy colleagues as patients move between care settings developing local effective communication, referral practices and procedures

Proactively engage with patients whose care has been transferred to reduce potential readmission

Maintain full and complete records of all patient contacts using appropriate clinical templates and SNOMED coding

Provide feedback and seek advice from the primary care team for patients where the post holder has clinical concerns around competence to safely manage their medications

Support the effective utilisation of the practice workforce

Governance, Compliance & Quality
Identify high-risk patients through data analysis.

Participate in audit cycles and complete full PDSA cycles.

Analyse prescribing trends to support evidence-based improvement.

Recall patients for medication monitoring where required.

Contribute to research and service development initiatives within The Garth Surgery

Digital & Operational Support
Utilise clinical systems e.g. TPP effectively.

Manage clinical correspondence pathology, discharge summaries, letters.

Use digital tools to optimise workflow and patient access.

Support review of incoming results and clinical communications in a timely manner.

Knowledge & Professional Development
Maintain CPPE PCPEP qualification or equivalent.

Maintain Independent Prescriber status where applicable.

Undertake mandatory training BLS, safeguarding, infection control.

Maintain a structured Personal Development Plan.

Participate in appraisal and clinical supervision.

Engage in continuous professional development in line with GPhC standards

Person Specification
Qualifications
Essential
  • GPC registered pharmacist
  • Proof of relevant professional and educational qualifications, coupled with details of any training
  • To be enrolled in, or have qualified from, an approved 18-month training pathway or equivalent that equips them to be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE clinical pharmacist training pathways)
  • Hold or be working towards an GPhC independent prescribing qualification
  • Working towards faculty membership of the Royal Pharmaceutical Society
  • Medicines management qualification
  • Minor ailments certification
  • MUR and repeat dispensing certification

Desirable
  • An appreciation of the nature of general practice and the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Experience in managing pharmacy services in primary care
  • Minimum of two years working as a pharmacist demonstrated within a practice portfolio
  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • Understanding of the mentorship process
  • An appreciation of the new NHS landscape, including the relationships between individual practices, PCNs and the commissioners
  • Experience of working autonomously
