Job summary
This post as Pathway Lead within Urgent Neighbourhood Services will work within the Leadership team of the Directorate. The services within the Directorate are Urgent Community Response, Hospital @ Home, Advanced Clinical Practitioners and the Digital Hub.

This post will work across the services offering operational and clinical leadership working closely with the Lead ACP and Directorate Manager.

The Pathway Lead is pivotal in ensuring high quality care is delivered throughout the Services.

Main duties of the job
  • Ensure the efficient and effective running of the services.
  • Supervise the provision and delivery of the pathway services by working in collaboration with Clinical Leads, Team Lead and team members.
  • Develop and implement operational management systems and process pathways specific for the individual services patients, in collaboration with services within and external to the Organisation.
  • Work alongside the Team Lead and/or Clinical Leads offering critical challenge within current practice, thus ensuring that all care delivered is evidence based, effective and efficient.
  • Ensure that all care provided by the team members is evaluated and reviewed and efficiency maximised.
  • Adopt a LEAN methodology and philosophy.
  • Lead with key stakeholders on the identification, development and evaluation of operational objectives and create the appropriate SOP's, protocols, and strategies to maintain efficiency and effectiveness.
  • Identify actual or potential operational/service risks, taking appropriate action, using when required the appropriate reporting structures, in accordance with Trust policies and guidelines.

Job responsibilities
  • Ensure patient involvement is integral to all service improvement activity.
  • Develop strong links with patient and carer groups identifying opportunities for involvement and engagement in any service redesign activity.
  • Ensure appropriate referral and care pathways across service areas are cohesive and interface effectively to ensure smooth transition of patients.
  • Work collaboratively with the service Clinical Leads thus ensuring that operational process is aligned to professional practice.
  • Ensure staff keep accurate and comprehensive patient records (written and electronic) in line with professional and Trust standards of practise, adhering to guidance and best practice

Person Specification
Education & Qualifications
Essential
  • Registered Professional RN/AHP
  • First level degree or equivalent experience
  • Management /leadership qualification or equivalent experience
  • Project management qualification or experience
  • Postgraduate study at advanced level, or equivalent experience
Desirable
  • Non Medical Prescribing V300
  • Coaching qualification or experience
  • Clinical examination/skills L7 or equivalent module or experience

Knowledge
Essential
  • Understanding of clinical risk and incident management, patient safety and clinical quality improvement initiatives
  • Awareness of clinical governance and impact on practice
  • Awareness of local and national issues

Experience
Essential
  • Experience of working in senior clinical or managerial role for 2yrs
  • Evidence of significant practical contribution to service development
  • Change management experience and direct involvement with service redesign
  • Evidence of managing complex patients, and advanced communication in stressful and emotional situations
  • Evidence of continuous professional development and practical application to work place
  • Caseload coordination and management
  • Experience of management of complaints and complaints processes
  • Ability to demonstrate solid understanding of Primary Care and secondary care interface
  • Commitment to the development of integrated working with Primary and Secondary care

Skills & Abilities
Essential
  • Documented evidence of advanced clinical skills and competencies
  • Excellent interpersonal skills with effective oral and written communication, with advanced communication skills and ability to initiate difficult and challenging conversations Confidence to discuss patient management with senior colleagues
  • Confidence and ability to accurately assess and manage complex patients

  • Flexible and adaptable in approach to work
  • Holds current driving licence and car owner with insurance for business use
  • Reliable work record
