We are recruiting for an Advanced Clinical Practitioner to support general practice in Walsall and in particular, the PCN and its objectives. The successful candidate will work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating treatment as appropriate. The post includes working within the Walsall practices.
The post will work with all the 5 clusters of GP surgeries known as Primary Care Networks across the North, East, South or West of Walsall. The post will be based at and provide the service from the GP surgeries, with some requirement to attend team meetings and training at OurNets Head Office, located in Bloxwich, as well as training at other local or regional venues as necessary.
ACPs:
Candidates will have a masters degree. Is working at a masters level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies. Has qualified from an approved 18-month training pathway or equivalent and qualified as an independent prescriber
Main duties of the job
  • To work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating treatment as appropriate.
  • To assess, diagnose and treat undiagnosed and diagnosed conditions for patients within a primary care setting.
  • To develop innovative concepts, models, methods and practices which deliver new and improved primary care services to meet the changing needs of primary care.
  • To trial and adopt new working methods designed to manage the increasing demand in primary care.
  • To provide education and training to other staff and students as appropriate and to take a lead role in encouraging teamwork and sharing best practice.
  • To undertake audit and research if and when required.
  • To promote healthy lifestyles, health promotion and disease prevention, by suggesting and recommending possible solutions.
  • Manage caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence

  • Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time eg. Pathology results, common/minor ailments, acute conditions, long term condition reviews, etc.

OurNet Health Services is a provider of primary care service to NHS patients in Walsall. Established in 2019 with the support of GPs in Walsall, we aim to deliver excellent care to patients when they need it most.
Job responsibilities
  • Overlook agreed targets and participate in regular supervision for all teams and ensure service is used effectively and proficiently.
  • Engage in training, identify personal and professional development needs and continuously seek to refresh knowledge in relation to the role.
  • Communicate with the wider team to ascertain funding and training opportunities.
  • Cooperate in the formulation and development of systems for monitoring and evaluating the work across all the organisations.
  • Provide information for and contribute to the administration of communications as appropriate.
  • Work within the Clinical Governance framework of the practices in the PCN, incorporating risk management and all other quality initiatives.
  • Treat with confidentiality any information about any organisation, its staff, trustees or clients that is sensitive, personal or private.
  • Cover the work of colleagues during holidays or absences.
  • Be aware of and adhere at all times to OurNet Health Services Ltds policies and procedures, including the organisations equal opportunities policies and with due regard for health and safety, and the relevant policies and procedures of the host Primary Care Network.
  • Work flexibly and be available for occasional out-of-hours working as appropriate.
  • Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.

Person Specification
Qualifications
Essential
  • Qualifications and Registrations
  • Reguistered Nurse (NMC) or Allied Health Professional with current registration (HCPC)
  • Masters - Advanced Clinical Practice (or working towards with substantial ACP experience)
  • Independent Prescriber
  • Understanding of national and local guidelines

Desirable
  • Primary Care Experience
  • Experience using EMIS
  • Knowledge of local care pathways and community services.

Experience
Essential
  • Management of patients with long-term conditions
  • Management of patients with complex needs
  • Clinical examination skills
  • Accountability of own role and other roles within the multi-disciplinary team
