Job summary
Spring Gardens Group Medical Practice is seeking an enthusiastic and motivated Advanced Clinical Practitioner (ACP) to join our experienced multidisciplinary team.

We are a well-established, forward-thinking GP training practice located in the heart of Worcester, providing high-quality primary care to over 20,000 patients. You will be joining an established team of Advanced Clinical Practitioners working alongside GPs, nurses, pharmacists, and other healthcare professionals to deliver excellent patient-centred care.

This is an exciting opportunity for an experienced ACP looking to work within a supportive, collaborative environment where professional development is encouraged and clinical excellence is valued.

Main duties of the job
The successful applicant will work autonomously within their scope of practice, providing assessment, diagnosis, treatment and management of patients presenting with acute and long-term conditions.
The role includes:
  • Assessing, diagnosing and managing patients with undifferentiated and diagnosed conditions.
  • Independently prescribing where appropriately qualified.
  • Managing acute presentations and same-day appointments.
  • Contributing to the management of patients with long-term conditions.
  • Working collaboratively with GPs and the wider multidisciplinary team.
  • Making appropriate referrals and coordinating patient care.
  • Maintaining accurate clinical records using the EMIS clinical system.
  • Participating in clinical governance, quality improvement and audit activities.
  • Supporting the education of learners within our GP training practice where appropriate.

Spring Gardens Group Medical Practice is a large, established GP training practice serving over 20,000 patients from our premises in central Worcester.

Our clinical team includes eight GP Partners, salaried GPs, Advanced Clinical Practitioners, nurses, pharmacists, and a highly skilled administrative team. Clinical discussion and peer support are an integral part of our culture, and our GP Partners are readily available to provide advice, supervision and collaborative decision-making whenever required.

As a training practice, we are committed to education, innovation and continuous professional development for all members of the team.

We are also heavily involved in research.
Job responsibilities
Job Role
The Advanced Clinical Practitioner (ACP) will work as an autonomous clinician within the multidisciplinary team, providing advanced clinical assessment, diagnosis, treatment and management of patients presenting with undifferentiated, undiagnosed and diagnosed conditions.

The post holder will demonstrate expert clinical decision-making skills, managing patients with acute, urgent and long-term conditions while working within their professional scope of practice and in accordance with the four pillars of Advanced Clinical Practice: Clinical Practice, Leadership and Management, Education, and Research.

The ACP will contribute to the delivery of safe, effective and high-quality primary care services, supporting the practice in meeting local and national priorities whilst promoting patient-centred care.
Key Responsibilities
Clinical Practice
The post holder will:
  • Undertake comprehensive clinical assessment, history taking and physical examination of patients presenting with acute, chronic and undifferentiated conditions.
  • Formulate differential diagnoses using advanced clinical reasoning.
  • Develop and implement evidence-based management plans.
  • Request, interpret and act upon appropriate diagnostic investigations.
  • Independently prescribe medication where appropriately qualified and competent.
  • Manage same-day urgent appointments and minor illness presentations.
  • Assess and manage patients with long-term conditions, including treatment optimisation and ongoing monitoring.
  • Recognise red flag symptoms and clinical deterioration, ensuring timely escalation and referral where appropriate.
  • Refer patients appropriately to secondary care, community services and allied professionals.
  • Provide health promotion, preventative care and lifestyle advice.
  • Maintain accurate, contemporaneous and legally compliant clinical records.

Professional Responsibilities
  • Practise within professional registration requirements (NMC or HCPC).
  • Work within the ACP framework and individual scope of competence.
  • Maintain professional registration and mandatory training.
  • Participate in annual appraisal and revalidation.
  • Maintain competence through continuing professional development.
  • Comply with practice policies, national guidance and evidence-based practice.

Education
  • Support the supervision and development of students, trainee ACPs and other learners where appropriate.
  • Share knowledge and best practice within the multidisciplinary team.
  • Participate in educational meetings and protected learning events.

Quality, Governance and Audit
  • Participate in clinical audit and quality improvement programmes.
  • Contribute to significant event reviews and learning from incidents.
  • Support clinical governance activities.
  • Ensure compliance with CQC standards and national guidance.
  • Participate in infection prevention and control activities.

Communication
  • Communicate effectively with patients, relatives and carers.
  • Work collaboratively with GPs, nurses, pharmacists and the wider multidisciplinary team.
  • Participate in multidisciplinary meetings where appropriate.

Information Governance
  • Maintain confidentiality at all times.
  • Comply with UK GDPR, the Data Protection Act and Information Governance requirements.
  • Ensure accurate documentation within the practice clinical system.

Safeguarding
  • Recognise and respond appropriately to safeguarding concerns involving children and vulnerable adults.
  • Follow local safeguarding policies and procedures.
  • Participate in safeguarding training appropriate to the role.

Health and Safety
  • Comply with all Health and Safety policies and procedures.
  • Report incidents, accidents and near misses appropriately.
  • Promote a safe working environment for patients, visitors and colleagues.

  • Deliver care without discrimination.

Other Duties
The post holder may be required to undertake other duties appropriate to the grade of the post, following discussion with the Partners or Practice Management Team, in line with service requirements.
Person Specification
Qualifications
Essential
  • Registered healthcare professional (NMC or HCPC).
  • Master's degree in Advanced Clinical Practice (or equivalent recognised qualification).
  • Demonstrable experience working autonomously as an Advanced Clinical Practitioner.
  • Excellent assessment, diagnostic and clinical decision-making skills.
  • Ability to manage patients presenting with acute illness.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively within a multidisciplinary team.
  • Commitment to high standards of patient care and continuous professional development.

Desirable
  • Independent Prescriber qualification.
  • Experience working within General Practice or Primary Care.
  • Diploma or postgraduate qualification in Diabetes Care.
  • Experience in the management of long-term conditions, particularly diabetes.
