Job description
Job summary
Position available up to full time, part time position considered. Provides safe, effective and evidence-based pharmaceutical care, including independent prescribing, to optimise patient outcomes. Works autonomously, applying advanced clinical knowledge and judgement to assess, diagnose and manage patients within scope of practice. Contributes to the delivery of clinical services, coordinating care across teams, applying policies and guidelines in practice, and supporting service improvement and quality of care.

The Key Medical Practice is owned and operated by Principal Medical Limited (PML), which works in partnership with the NHS to deliver primary care services in GP practices and community settings. Founded in 2004 as a not-for-profit organisation by local GPs, PML was established to enable healthcare professionals to develop and deliver innovative services at scale. Today, 70% of GP practices in Oxfordshire and 22% in Northamptonshire are associated with the Federation. PML is the largest GP Federation in Oxfordshire and South Northamptonshire, serving nearly 800,000 patients. We believe local providers are best placed to deliver high-quality, cost-effective care that meets community needs. With over 20 years of experience, PML has a strong track record of delivering NHS, ICB and Public Health services. Role Accountabilities
Clinical Assessment and Prescribing
  • Contribute to the delivery of safe, effective and evidence-based clinical assessment, diagnosis and prescribing within scope of practice.
  • Apply advanced clinical knowledge and judgement to determine appropriate treatment plans, including medicines optimisation and deprescribing where required.
  • Manage a defined caseload, prioritising patients based on clinical need and service requirements.
Medicines Optimisation and Safety
  • Contribute to safe and effective use of medicines, ensuring prescribing aligns with clinical guidelines and best practice.
  • Identify and respond to medication-related risks, escalating complex or high-risk situations appropriately.
  • Support safe prescribing practices and contribute to minimising medicines-related harm.
Patient Care and Engagement
  • Provide patients with clear advice, education and support to enable informed decision-making and adherence to treatment.
  • Use communication and influencing skills to promote behaviour change and optimise therapeutic outcomes.
Multidisciplinary and Service Collaboration
  • Work collaboratively with multidisciplinary teams, contributing to coordinated patient care and clinical decision-making.
  • Support integration of prescribing activity across services and care pathways.
  • Build effective working relationships with colleagues, patients and stakeholders to support service delivery.
Governance, Compliance and Quality
  • Contribute to the application of clinical governance, including audit, risk management, safeguarding and incident reporting.
  • Maintain accurate, timely and compliant clinical records in line with legal and professional standards.
  • Support continuous quality improvement and service development initiatives.
Professional Practice and Development
  • Maintain professional competence, including prescribing practice, through supervision, reflective practice and continuing professional development.
  • Keep up to date with relevant clinical guidance, legislation and best practice, applying these in day-to-day work.
  • Contribute to audit, training and small-scale service improvement activity where appropriate.

Person Specification
Experience
Essential
  • Experience delivering clinical pharmacy services in a patient-facing healthcare setting
  • Experience supporting safe medicines management and clinical governance processes
Desirable
  • Experience within urgent care, primary care or out of hours services.

Skills and Knowledge
Essential
  • Strong clinical knowledge of therapeutics and medicines optimisation, with sound clinical assessment and decision-making skills
  • Knowledge of medicines legislation, regulatory standards and risk management within healthcare
Desirable
  • Knowledge of NHS commissioning structures and local formulary processes

  • Demonstrates professionalism, integrity and accountability in clinical practice
  • Effective communication and collaborative working skills within a multidisciplinary team
Desirable
  • Proactive and improvement focused approach, with commitment to continuous professional development.

Qualifications
Essential
  • Master of Pharmacy (MPharm) degree or equivalent qualification
  • Registered Pharmacist with current General Pharmaceutical Council (GPhC) registration.
Desirable
  • Independent Prescriber qualification or working towards this
