Main duties of the jobThe successful candidate will provide expert, autonomous clinical care within a busy and supportive general practice, managing patients with a broad range of acute, urgent and long-term conditions. Working as an integral member of our multidisciplinary team, you will assess, diagnose, treat and, where appropriate, prescribe independently, ensuring patients receive timely, high-quality care. You will play a key role in improving access to primary care by managing your own clinical workload, making evidence-based decisions and referring patients appropriately when required. In addition to direct patient care, you will contribute to health promotion, preventative care, clinical governance, quality improvement initiatives and service development. Working closely with GPs, Practice Nurses, Clinical Pharmacists, First Contact Practitioners and other healthcare professionals, you will help deliver safe, effective and person-centred care while supporting the ongoing development of our clinical services. Job PurposeThe Advanced Clinical Practitioner (ACP) will provide autonomous, high-quality, patient-centred clinical care within Caritas Group Practice. Working within their scope of practice, the ACP will assess, diagnose, treat and manage patients presenting with undifferentiated and diagnosed conditions, using advanced clinical reasoning and evidence-based practice. The ACP will work collaboratively with GPs and the wider multidisciplinary team to improve access to primary care, deliver safe and effective clinical services, support quality improvement initiatives and contribute to the ongoing development of the practice.
Main Duties and ResponsibilitiesClinical Responsibilities- Assess, examine, investigate, diagnose and manage patients presenting with acute, urgent and long-term conditions.
- Independently prescribe medications in accordance with current legislation and best practice (where qualified).
- Order, interpret and act upon diagnostic investigations including pathology and imaging.
- Formulate appropriate management plans and provide follow-up where required.
- Refer patients appropriately to secondary care and community services.
- Provide clinical advice, health education and health promotion to patients and carers.
- Undertake home visits where appropriate.
- Recognise and manage clinical risk, safeguarding concerns and medical emergencies.
- Maintain accurate, timely and contemporaneous clinical records using SystmOne.
- Work within national and local clinical guidelines and practice protocols.
Service Delivery- Manage a defined clinical workload, including same-day, urgent and routine appointments.
- Support effective patient flow and improved access to services.
- Work collaboratively with GPs, Practice Nurses, Healthcare Assistants, Clinical Pharmacists, First Contact Physiotherapists, Mental Health Practitioners, Social Prescribers and other members of the multidisciplinary team.
- Participate in multidisciplinary team meetings where appropriate.
- Contribute to service redesign and the implementation of new models of care.
Clinical Governance- Maintain high standards of clinical practice and professional conduct.
- Participate in clinical audit, quality improvement and service evaluation.
- Contribute to significant event reviews and clinical governance activities.
- Support the implementation of national guidance and local policies.
- Ensure compliance with infection prevention and control procedures.
- Maintain patient confidentiality and comply with GDPR and information governance requirements.
Leadership and Professional Responsibilities- Demonstrate leadership within the clinical team.

- Master's degree in Advanced Clinical Practice (or working towards completion with an agreed timeframe).
- Independent Prescriber qualification.
- Evidence of advanced clinical assessment and diagnostic skills.
- Evidence of ongoing Continuing Professional Development (CPD).
- Current Basic Life Support (BLS) certification.
Desirable- Advanced Life Support (ALS) certification.
- Teaching, mentorship or clinical supervision qualification.
