Job description
Job summary
This role helps people with urgent health needs by assessing, diagnosing and treating a wide range of conditions, making a real difference to timely patient care. You'll work as part of a skilled urgent care team, supporting patients with both minor illness and more complex needs. You will practise with a high level of autonomy while collaborating closely with colleagues across health and care services. Your work will help ensure safe, effective care and smooth patient journeys.

Main duties of the job
What you'll do:
  • Carry out telephone triage and face-to-face assessments for undiagnosed conditions
  • Diagnose, treat and manage patients, including referral or discharge
  • Prioritise and respond to urgent and emergency presentations
  • Work with multidisciplinary teams to coordinate safe care pathways
  • Support and supervise junior staff and students

What we're looking for:
  • Current professional registration with NMC or HCPC
  • Degree in nursing, paramedic science or equivalent experience
  • Advanced assessment skills and evidence-based clinical decision making
  • Experience in urgent or unplanned care settings
  • Ability to work independently and as part of a team, with strong communication skills

Qualifications
Essential
  • Professional registration with the NMC (Nursing and Midwifery Council) or the HCPC (Health and Care Professions Council).
  • Approved degree in nursing or paramedic science or equivalent experience.
  • Post graduate qualification in relevant subject or equivalent experience
  • Teaching/Mentoring qualification
  • APACS (Advanced Physical Assessment and Consultation Skills) or equivalent qualification/experience
  • Non-medical prescribing qualification

Knowledge
Essential
  • Highly specialised knowledge of relevant policy and legislation in relation to unplanned care
  • Working knowledge of multi-disciplinary teams
  • Evidence of Continuing Professional Development
  • Specialised knowledge of medication and interventions used in unplanned care
  • Demonstrate the use of evidence-based practice
  • Knowledge of the legal framework surrounding medications prescribed or supplied, including PGDs, PSDs, EOL care pathways etc.

Experience
Essential
  • Experience in a specialist environment, demonstrating effective leadership and autonomy.
  • Experience of multi-disciplinary team working and working with patients with minor injuries and minor illness presentations
  • Experience of the initial management, stabilisation and onward referral of patients who may self-present with more serious/life threatening conditions
  • Previous experience of producing reports or audits
  • Experience of utilising a variety of software packages e.g. SystmOne, and E-Mail,
  • Ability to prioritise effectively in a clinical environment
  • Previous leadership and management experience within a clinical setting
  • Experience and competency in supervision and support of others including the prioritisation of patient care e.g. junior staff, non-registered staff and students

Desirable
  • Experience of using PGDs
