Job description
Newgate Medical Group is seeking an experienced Locum Clinical Pharmacist to join our supportive and forward-thinking primary care team. This flexible locum opportunity offers the chance to work within a busy, patient-focused practice, providing high-quality clinical pharmacy services alongside experienced GPs, nurses, pharmacists and other healthcare professionals. You will play a key role in supporting medicines optimisation, delivering structured medication reviews, managing long-term conditions and improving patient outcomes, while enjoying the flexibility of locum work within an established multidisciplinary team. Whether you are looking for regular sessions or more flexible arrangements, we would be pleased to hear from you. Main duties of the job As a Locum Clinical Pharmacist, you will play a key role within our multidisciplinary primary care team, providing expert clinical pharmacy services to improve patient outcomes and support safe, effective prescribing. You will undertake structured medication reviews, manage long-term conditions, optimise medicines, and provide specialist advice to patients and healthcare professionals. Working closely with GPs, nurses, community pharmacies and secondary care, you will help ensure seamless medicines management across care settings. The role also includes supporting quality improvement initiatives, promoting evidence-based practice, contributing to service development, and providing education and support to colleagues. We have an established team of 9 GP Partners & a host of varied health professionals including pharmacists, nurses, ANPs, PA, Paramedics, Social Prescribing linkworker, care co-ordinators & a large admin team. We offer a welcoming working environment with modern facilities, experienced prescribing support and well-established clinical systems Job responsibilities The following key responsibilities in delivering healthservices at Newgate Medical Group.There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Primary responsibilities: * Provide patient-facing clinical pharmacyservices as part of the multidisciplinary team. * Assess, treat and prescribe within professional competence. * Carry out structured medication reviews and optimise medicines for patients with long-term conditions and complex needs. * Support medicines optimisation, antimicrobialstewardship and quality improvement initiatives. * Promote safe, effective and cost-efficientprescribing. * Manage repeat prescribing, medication monitoringand high-risk drug recalls. * Respond to medicines-related queries frompatients and healthcare professionals. * Conduct medication reviews for newly registeredpatients, care home residents and housebound patients. * Deliver long-term condition clinics andpharmaceutical consultations. * Work collaboratively with GPs, communitypharmacies, hospitals and wider healthcare teams. * Lead on medicines management processes andprescribing systems. * Maintain accurate clinical records and complywith professional, legal and safeguarding requirements. * Support clinical audit, service improvement andimplementation of national guidance (including NICE). * Supervise the safe storage, handling anddisposal of medicines and vaccines. * Contribute to enhanced services, public healthinitiatives and clinical governance. * Undertake other duties appropriate to the roleas required. In addition to the primary responsibilities, the ClinicalPharmacist will: * Support the delivery of QOF, QIPP, incentives chemes and other quality improvement initiatives. * Review prescribing formularies, protocols andcompliance. * Improve the quality of medicines data andclinical coding. * Work with hospital and community pharmacy teamsto improve medicines safety and continuity of care. * Develop and share medicines information throughnewsletters, bulletins and education. * Develop and maintain a specialist area ofclinical expertise. * Support digital models of care, including remoteconsultations and medication reviews. * Undertake duties appropriate to the role andcontribute to the effective delivery of the service. Person Specification Qualifications * GPhC registered pharmacist. * To be enrolled in, or has qualified from, an approved 18-month training pathway or equivalent that equips them to be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE clinical pharmacist training pathways). * Hold or be working towards an GPhC independent prescribing qualification. * Proof of relevant professional and educational qualifications, coupled with details of any training. * Working towards faculty membership of the Royal Pharmaceutical Society * Minor ailments certification * MUR and repeat dispensing certification Clinical Knowledge and Skills * Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face. * Excellent interpersonal, influencing, negotiation and organisational skills with the ability to constructively challenge the views and practices of managers and clinicians. * Understanding of safeguarding adults and children. * Ability to promote best practice regarding all pharmaceutical matters. * Ability to work within own scope of practice and understanding when to refer to GPs. * Ability to record accurate clinical notes. * Good clinical system IT knowledge. * Plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions. * Broad knowledge of clinical governance. Personal Qualities * Effective time management (planning and organising). * Ability to follow legal, ethical and professional policies/local policies/procedures and codes of conduct. * Ability to use own initiative, discretion, and sensitivity. * Ability to identify risk and assess/manage risk when working with individuals. * Demonstrate personal accountability, emotional resilience and work well under pressure. * Knowledge of IT systems including the ability to use word processing skills, emails and the internet. * Problem solving and analytical skills. * Knowledge of and ability to work to key policies and procedures. * Ability to work as a team member and autonomously. * Commitment to ongoing professional development. * Good interpersonal and organisational skills. * Sensitive and empathetic in distressing situations. * Clear, polite telephone manner. Experience * An appreciation of the nature of general practice and the nature of primary care prescribing, the concepts of rational prescribing and strategies for improving prescribing. * Minimum of two years working as a pharmacist demonstrated within a practice portfolio. * An appreciation of the new NHS landscape, including the relationships between individual practices, PCNs and the commissioners. * Experience of managing pharmacy services in primary care. * In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare. * Understanding of the mentorship process. * Experience of working autonomously.
