Job description
We are seeking a highly skilled and autonomous Advanced Nurse Practitioner (ANP) to join our Team. This role is pivotal in providing high-quality, patient-centred care in a fast-paced, supportive general practice environment. As an ANP, you will lead on delivering undifferentiated primary care to a varied patient population, working independently but with access to clinical support from the practice team. The successful candidate will demonstrate excellence in clinical assessment, decision-making, and prescribing, with a commitment to upholding the standards of care and safety expected by Gateshead Healthcare. Main duties of the job Key Responsibilities The Advanced Nurse Practitioner (ANP) will utilise advanced clinical knowledge and skills to independently assess, diagnose, treat, prescribe, and provide expert advice to patients through face-to-face consultations, delivering evidence-based care that is safe, effective, and patient-centred. The role includes responsibility for: Delivering undifferentiated patient care during morning and afternoon clinical sessions. Providing timely follow-up of patients and responding appropriately to ongoing case management requirements. Practising autonomously within professional competence, adhering to current NICE guidance, national standards, and best practice. Managing complex patient cases, ensuring appropriate clinical pathways are followed and tailored to meet the needs of both patients and the service. Independently prescribing medications within scope of practice, ensuring safe, evidence-based prescribing in accordance with current legislation and professional standards. Job responsibilities Clinical Responsibilities Deliveradvanced and complex clinical care, providing assessment, diagnosis, treatment,and prescribing for patients with acute, chronic, and complex conditions. AutonomousPractice Work independently within the scope of practice to manage completeclinical episodes, using advanced diagnostic skills and clinical judgment. Case Management Manage patient cases from initial assessment to conclusion, includingfollow-ups and referrals, ensuring continuity of care. Prescribing Utilise prescriptive authority responsibly following current guidelines andprotocols to enhance therapeutic outcomes for patients. Home visiting Undertake home visiting as and when required, including triage of home visits. Leadership and Guidance Provide mentorship and supervision to nursing staff, trainee advancedpractitioners, and other healthcare professionals to enhance team competencies. ProfessionalDevelopment Engage in continuous professional development to keep abreast ofmedical advancements and to further develop clinical skills. Research and QualityImprovement Implement quality improvement initiatives and ensure compliance withhealth care standards and best practices in patient care. Educational Activities CPD Activities Facilitate and participate in Continuous Professional Development (CPD)activities within the practice. Administrative andOrganisational Duties Record Keeping Maintain comprehensive and accurate patient records using EMIS Web. The Practice recognisesthat the role of an Advanced Nurse Practitioner is wide and varied isevolving in nature. Therefore, this job description is not intended to beexhaustive and there will be other tasks and duties that the post holder willbe expected to perform within what is ordinarily expected of this role. Person Specification Experience * Extensive experience in managing complete episodes of clinical care including assessment, diagnosis, treatments, and prescribing * Advanced clinical skills in a primary care setting, with proficiency in using clinical diagnostic tools and conducting complex clinical procedures Qualifications * Masters degree in advanced Practice, or equivalent post-graduate qualification, or full accreditation or eportfolio * Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) or other regulatory body as appropriate * Further qualifications in a specialist area such as diabetes. * Experience and competency in chronic disease management.
