Job description

Community Physiotherapist The closing date is 12 July 2026. This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly. An opportunity has arisen for a Physiotherapist to join the Community rehab team currently based out of St Austell Community Hospital. The successful post holder will initially be covering a community rehab caseload and be fluid in where they work to meet ever‑changing service needs. The team treats a range of physical conditions, assessing and treating patients in their own homes. You will join a team of both physiotherapists and occupational therapists and share an office, creating a supportive learning environment. Main duties of the job * Provide specialist assessment, effective treatment and individual goal setting of patients in their own home/place of residence. * Work alongside a wider MDT within the community including partner agencies, liaising with community and acute hospitals, adult social care and voluntary workers. Over 5,000 people make up the Trust, including doctors, nurses, therapists, plus admin and support staff. We work in peoples homes, in community clinics and bases. Some staff work from one of our 10 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner. Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays many people choose to visit the area. This increases the number of people who use our services. Job responsibilities You will be expected to carry out specialist assessment of patients as an autonomous practitioner including those with highly complex presentations, and to use advanced clinical reasoning skills and knowledge of evidence‑based practice to determine appropriate diagnosis and care plan utilising a wide range of treatment skills and options. This is an ideal opportunity for a physiotherapist keen to develop his/her rehabilitation skills in the community. We will offer a supportive working environment in which to develop your clinical and interpersonal skills, a structured in‑service training programme; appraisal and supervision system. You will be encouraged to supervise support workers and more junior staff, and be involved in the clinical education of students. Person Specification Education / Qualifications and Relevant Experience * Degree or Diploma in Physiotherapy * HCPC Registered * Broad range of post‑graduate experience in medical, surgical, respiratory, orthopaedic, and neurological settings Skills and Aptitude * High level of assessment and treatment skills Knowledge and abilities * Ability to communicate effectively with patients and others concerning Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
