Your clinical expertise will support patients with acute illness, long-term condition management, preventative healthcare and complex care needs, helping improve health outcomes and patient experience.Working within EMIS Web and supported by experienced multidisciplinary colleagues, you'll contribute to continuity of care through clinical decision-making, prescribing, referrals, follow-up and collaborative care planning.You'll work within a mutually agreed job plan, developed collaboratively to support high-quality patient care, professional fulfilment and sustainable ways of working.
