Lead Nurse - Wandsworth End of Life Care CoordinationService (WEoLCCS) Salary: AfC Band 6 Hours: 37.5 hours per week. Including weekends and publicholidays An excitingopportunity has arisen to join our WEOLCCS team as a Lead Nurse. Would youenjoy a role working within a team that makes a real difference? This role plays avital part in the goal of the end-of-life care system in Wandsworth, supportingpatients to achieve their preferred place of care and death, to avoid unplannedand inappropriate hospital admissions and to support rapid discharges fromhospital. The nature of therole will require you to use your initiative and will involve interactions andmanaging the wide range of complexities presented by patients, their carers andfamilies. Main duties of the job * To be responsible for all aspects of the day-to-day operationalLeadership and management for a team of Royal Trinity Carers and CareCoordinators. * To act as an End-of-life Care champion, build and maintain professionalrelations (internally and externally) to promote service. * To work closely with all service delivery teams, Continuing Health Careand care agencies to ensure care is planned to meet the unique and changingneeds of patients, their families, and friends. * To provide an expert clinical resource to the team, the organisation,and external stakeholders. * To support the Head of Service in developing and implementing strategicgoals and direction. The main responsibilities of the Lead Nurseare: * To act as a communitynursing end of life champion and inspire others demonstrating a range of skills * To directly manage and lead the Services Health& Personal Care Delivery Team * To provide clinical support to members of theService * To work closely withthe Services care delivery team, Continuing Health Care, and the care agenciesto ensure care is planned to meet the unique and changing needs of thepatients, their families and friends * To build and maintain professional relationships(internally and externally) and to promote Trinitys services * To work flexibly within the Clinical Hub Servicesteam when there is a need Person Specification Qualifications * Registered with the NMC * Evidence of Continuing Professional Development * Knowledge of best practice in assessing end of life care needs * Community Nursing Experience * Mentorship and teaching qualification Experience * Registered with the NMC in Adult Nursing * Previous experience of managing staff * Evidence of sound knowledge in the assessment of care needs and planning interventions * Excellent verbal and written communication skills * Excellent interpersonal skills * Ability to work as part of the interdisciplinary team as well as on own initiative * Community Nursing Experience Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
