Advanced Practitioner Social Worker - Safeguarding Location: Taunton, Somerset Pay Rate: £44.00 per hour (Umbrella) / £32.13 per hour (PAYE) Working Pattern: Hybrid Working – 1 day per week in the office (Wednesday at County Hall, Taunton) Key Responsibilities * Provide professional leadership and support within Adult Social Care services. * Promote wellbeing, independence, and person-centred practice in line with the Care Act. * Complete timely and robust assessments, reviews, and support plans. * Safeguard vulnerable adults and manage complex safeguarding enquiries. * Support individuals to remain independent within their communities for as long as possible. * Work collaboratively with health professionals, families, carers, and partner agencies. * Facilitate safe and timely hospital discharges and help prevent unnecessary admissions. * Ensure services enhance existing informal support networks and community resources. * Support people to achieve outcomes that are important to them while maintaining choice and control over their care and support. Requirements * Qualified Social Worker. * Registered with Social Work England. * Significant experience working within Adult Social Care. * Strong knowledge of safeguarding legislation, policy, and practice. * Experience managing complex cases and undertaking risk assessments. * Excellent communication, decision-making, and leadership skills.
