Job description

We are looking for full or part time experienced, enthusiastic & compassionate ANP to join our clinical team. Weare a happy established clinical team that is looking for the right nurse tojoin us. Previous experience ofworking in General Practice is desirable; however, we are able to provide aTransition Programme for those who are new to Primary Care, if required. Main duties of the job The post holder will work as an autonomous clinician within the GPpractice, providing advanced clinical care to patients presenting with acute,chronic, and complex health needs. The role requires independent prescribing,advanced clinical assessment skills, and the ability to manage patients acrosstheir lifespan within a primary care setting. The ANP/ECP will workcollaboratively with GPs, nurses, and the wider multidisciplinary team toensure safe, effective, and patient-centred care, contributing to servicedevelopment and quality improvement within the practice. About us We prideourselves in providing the highest quality of traditional General Practice carein a supportive team environment. We are a friendly team consisting of 3 GPs, 7 ANPs, 1 ECP, 6 Practice Nurses, 1 NA, 2 HCAs, 1 GPA and a supportive adminstaff caring for 14,800 patients in a semi-rural setting. We are also supported by PCNARRS staff including Clinical Pharmacists, First Contact Physiotherapist,Social Prescriber and Health Coaches. Job responsibilities Main Duties and Clinical Responsibilities Undertakecomprehensive clinical assessments, including history taking, physicalexamination, and diagnosis Manageundifferentiated and differentiated presentations independently assess,diagnose, treat, and discharge or refer patients as appropriate Prescribemedications independently in line with clinical guidelines and best practice Manageacute, urgent, and long-term conditions Order, interpret, and act upon diagnostictests and investigations Providehealth promotion, disease prevention, and patient education Provide same-day urgent care appointments androutine clinical appointments as required Referpatients to Secondary Care, community services, or allied health professionalswhere appropriate Ensurecontinuity of care through appropriate follow-up and safety-netting Maintainaccurate, clear, and contemporaneous clinical records Multidisciplinaryworking -work collaboratively with GPs, practice nurses, HCAs, pharmacists, andthe wider PCN team Participate in MDT meetings and clinical discussions Supportand mentor junior staff where appropriate Quality, governance, and safety work withinpractice policies, procedures, and clinical governance frameworks, adhere tosafeguarding policies for children and vulnerable adults Participatein clinical audit, quality improvement, and service development initiatives Maintainprofessional registration, revalidation, and mandatory training Personal ProfessionalDevelopment Maintain and develop clinical competence through CPD Keep up todate with evidence-based practice and national guidance Actively contribute to a learning andsupportive practice environment Responsibility for Information GovernanceEnsure patient confidentiality is maintained at all times comply with GDPR,data protection, and Caldicott principles Useclinical systems appropriately and securely Health& Safety Work in accordance with Health & Safety legislation andpractice policies report incidents, near misses, and risks appropriately. The post holder may be required to undertakeadditional duties appropriate to the role, following discussion with the linemanager Person Specification Qualifications * Registered Nurse with the NMC * Qualified ANP * Independent prescriber * Managing acute and same-day presentations * Routine patient reviews and clinics * Supporting long-term condition management Skills * Ability to assess and manage patient risk effectively and safely * Well developed word processing/data collection/IT skills * Excellent interpersonal, verbal and written communication skills * Time management and ability to prioritise workload * Able to analyse data and information, drawing out implications for the individual patient/impact on care plan * Able to establish and maintain effective communication pathways within the organisation, the local PCT and with key external stakeholders * Experience of use of SystmOne * Proven record of effective use of networking and influencing skills * Ability to think strategically * Experience of presenting information to wider audience Knowledge * Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance. * Understanding of systems to gain an understanding of the health needs of the practice population as they relate to primary care * Understanding of evidence based practice * Knowledge of national standards that inform practice (eg, NICE guidelines etc) * Understanding of accountability arising from the NMC Code of Professional Conduct (2004) and medico-legal aspects of the Nurse Practitioner role * Understanding of equal opportunity and diversity issues Experience * Experience of working within a nursing team * Experience of dealing with vulnerable patients * Experience working in Primary Care * Good understanding of safeguarding adults and children * Knowledge of GP systems, QOF and enhanced services Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
