We are excited to announce the opportunity for a Qualified or Trainee ACP to join our T&O team in the division of NMSK.

We are looking for a driven, enthusiastic, dynamic individual to drive improvement in Osteoporosis management through autonomous ACP led clinics and to take an active role in leading service development.

The post holder will need to be able to work independently within the four pillars of advanced practice (clinical practice, leadership, education & research)

The candidate will have excellent leadership & clinical skills, as well as evidence of continuing professional development. The candidate will have drive, ambition, passion, a flexible approach and be dedicated to working in a fast paced, challenging but exciting environment.

This is a Band 7 Training post OR a Band 8A fully qualified ACP post.

The successful candidate will join and work alongside the ortho geriatric consultant team to gain the necessary clinical experience that will allow safe, autonomous evidenced based practice when fully qualified and accredited. The Trainee will be supported in completing the ACP accredited programme at MSc Level 7 at the local HEI, on a 1 - 3 year apprenticeship (dependent on existing modules). This will include 20% off the job time along with an engaged Consultant Clinical Education Supervisor.

0.8 WTE - 30 hours per week with 20% off the Job time allocated.

Main duties of the job

• Deliver telephone and in person older person osteoporosis clinics
• Make treatment plans, counsel patients, coordinate and prescribe osteoporosis treatments for outpatients and inpatients
• Work closely with the geriatric trauma consultants, Fracture Liaison Service (FLS), hip fracture nurse specialists and bone health for older people (BHOOP) clinic team
• Provide clinical continuity across outpatient, FLS and inpatient pathways by liaising with existing teams
• Strengthen links between multi-faceted services (Fracture Liaison Service, outpatient osteoporosis clinics, ward-based osteoporosis care on geriatric trauma wards as well as complex care/acute frailty/short stay medical wards) improving pathway coordination and outcomes
• Take an active role in service leadership and development.

Job responsibilities
Your role will involve the independent and autonomous practice of history taking, clinical examination, clinical investigations and interpretation and subsequently the diagnosis and management planning of the diagnosed patient as well as the undifferentiated and undiagnosed conditions.

Demonstrating a high level of expertise within all areas of Osteoporosis prevention and management. You will manage and co-ordinate care pathways for patients and will be expected to provide autonomous clinical review of patients in clinic and on the in patient wards. You will have expertise in coordinating patient care pathways, patient education and supporting patients and families on their recovery journey.

With a strong focus on quality improvement, the post-holder will lead and support on service development initiatives within the service to include education, research and audit promoting patient safety and patient experience. Within the role, there will be opportunity to develop autonomous ACP Led clinics providing comprehensive clinical assessment to improve symptom management, patient education and overall patient-centered care.

Essential

• Professional Registration
• Post graduate qualification in specific specialty or equivalent experience

Desirable

• Msc advanced practice modules - PACR, PADRAP, NMP

Work Experience
Essential

• Substantial clinical experience within T&O at Band 7
• Evidence of teaching

Desirable

• Able to demonstrate specialist knowledge in osteoporosis
