Job Title: Senior Reablement Enabler

Job Purpose

The Senior Enabler is responsible for supporting adults to regain, maintain, and maximise their independence through the delivery of high-quality Reablement services. Working within a strengths-based and person-centred framework, the post holder will support individuals with a range of needs, including older people, individuals with physical disabilities, sensory impairments, learning disabilities, and mental health needs.

The role involves completing risk assessments, coordinating and monitoring support services, delivering personal care and rehabilitation support, and working collaboratively with multidisciplinary professionals to help people achieve their agreed goals and live independently in their own homes or the place they call home.

The Senior Enabler will act as a role model for best practice, provide guidance to less experienced staff, contribute to service coordination, and ensure that all care and support are delivered in accordance with relevant legislation, safeguarding requirements, and organisational policies.


Key Responsibilities

Reablement and Support Delivery

  • Deliver high-quality Reablement support that promotes independence, wellbeing, and recovery.
  • Provide personal care, practical assistance, domestic support, and mobility assistance in accordance with individual Reablement plans.
  • Support individuals to develop confidence and skills required to live independently.
  • Assist service users with activities of daily living, including personal hygiene, meal preparation, medication support, mobility, and domestic tasks.
  • Support individuals in completing therapy-led exercise programmes and rehabilitation activities.

Assessment and Risk Management

  • Complete initial risk assessments in service users' homes and rehabilitation environments.
  • Identify environmental, personal, and safeguarding risks and implement appropriate control measures.
  • Monitor and review progress against agreed outcomes and goals.
  • Act as a Trusted Assessor (or undertake training to become one) to assess and prescribe simple equipment, minor adaptations, and assistive technology.

Care Coordination and Staff Support

  • Allocate and schedule work for team members, including evenings and weekends.
  • Assist with rota management and arranging cover for annual leave and staff absences.
  • Provide guidance and support to less experienced colleagues.
  • Monitor the quality of care and support being delivered and identify areas for improvement.

Partnership Working

  • Work collaboratively with social workers, occupational therapists, physiotherapists, nurses, GPs, housing teams, carers, families, and community organisations.
  • Attend and contribute to multidisciplinary meetings, reviews, and case discussions.
  • Promote coordinated, person-centred support to achieve the best outcomes for service users.

Safeguarding and Compliance

  • Ensure compliance with safeguarding policies and procedures.
  • Report safeguarding concerns immediately in line with Adult Safeguarding procedures.
  • Promote dignity, choice, equality, diversity, and inclusion in all aspects of service delivery.
  • Adhere to the Care Act 2014, Mental Capacity Act, GDPR, Health and Safety legislation, and relevant CQC standards.
  • Maintain excellent standards of infection prevention and control.

Administration and Record Keeping

  • Maintain accurate, timely, and comprehensive records of support provided.
  • Complete written and electronic documentation in accordance with organisational standards.
  • Record progress against Reablement goals and communicate updates appropriately.
  • Manage duty telephone responsibilities and respond effectively to enquiries and requests for support.

Service Development

  • Participate in supervision, training, appraisal processes, and continuous professional development activities.
  • Contribute to achieving team objectives and service outcomes.
  • Promote the Council's CARE values and commitment to customer service excellence.


Essential Qualifications

  • NVQ Level 3 in Health and Social Care, QCF Level 4, or an equivalent qualification.
  • Alternatively, willingness to undertake and complete the required qualification within agreed timescales.


Essential Experience

  • Minimum of two years' experience working within Adult Social Care, domiciliary care, community care, or a related health and social care setting.
  • Experience supporting adults with complex needs, disabilities, mental health conditions, or age-related needs.
  • Experience working within a Reablement, rehabilitation, enablement, or community-based care environment.
  • Experience applying strengths-based and person-centred approaches.
  • Experience supporting or mentoring less experienced staff.
  • Experience managing challenging situations, crises, or complex care needs.
  • Experience working effectively with diverse communities and individuals from different cultural backgrounds.


Knowledge, Skills and Competencies

Knowledge

  • Understanding of Reablement principles and the promotion of independence.
  • Knowledge of safeguarding adults and Making Safeguarding Personal.
  • Knowledge of the Care Act 2014, Mental Capacity Act, and relevant social care legislation.
  • Awareness of CQC standards and regulatory requirements.
  • Understanding of infection prevention and control procedures.

Skills

  • Strong assessment and risk management skills.
  • Ability to complete accurate records and electronic documentation.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong organisational and time management skills.
  • Ability to prioritise workload and respond effectively to changing demands.
  • Ability to support medication management in line with care plans.
  • Competent use of IT systems, smartphones, apps, and electronic recording systems.
  • Ability to support safe moving and handling activities using specialist equipment where required.


Special Requirements

  • Enhanced DBS clearance required.
  • Willingness to work flexibly across a 7-day service, including evenings, weekends, and where required, night support.
  • Ability and willingness to travel across the borough and service locations.
  • Ability to work in challenging environments, including individuals' homes where there may be hoarding, self-neglect, mobility constraints, or pets present.
  • Ability to meet the physical demands of the role, including moving and handling activities.

· Pay Rate: £26.83 per hour (Limited Company/Umbrella)

· Working Pattern: Flexible shift pattern across a 7-day service, including evenings, weekends, and where required, night support.

· Payment Schedule: Bi-weekly

· Start Date: Immediate start available
