Job description
Job summary
We are seeking a motivated and experienced Practice Nurse to join our clinical team. You will play a vital role in delivering person-centred care across a range of nursing services, including chronic disease management, immunisations, and health promotion. In joining the nursing team you will work closely with the GPs, and HCAs and be able to work autonomously within our multidisciplinary team

Main duties of the job
As a practice nurse you will play a crucial role in providing nursing care and treatment within our practice working alongside our clinicians.
The role encompasses a wide range of responsibilities, from managing chronic conditions to providing health education and screening.

We currently serve a list of over 14,000 patients with a team that includes GPs, nurses, pharmacists, healthcare assistants, physician associates, social prescribers, and wellbeing coaches. Our environment promotes learning, development, and collaborative practice.

Job responsibilities
The main duties include but are not limited to:
  • Administering vaccinations and immunisations in line with national and practice guidelines
  • Cervical cytology
  • Managing and monitoring patients with chronic diseases (e.g. asthma, COPD, diabetes, hypertension) in line with national and local guidelines
  • Carrying out home visits for housebound patients
  • Supporting the clinical team in the management of the care homes
  • Ensuring accurate and timely clinical record keeping and documentation
  • Conducting contraception reviews and coil checks
  • Carrying new patient and NHS health checks
  • Wound care and management including dressing, removal of sutures and clips
  • Participating in infection control, cold chain maintenance, ensuring fridge temperatures are regularly monitored and maintained
  • Supporting patients with lifestyle advice and self-care management
  • Advising patients on health care, minor ailments, health promotion, and illness prevention
  • Contributing to achievement of QOF (Quality and Outcomes Framework) and local enhanced services achievement and clinical audits
  • Preparing and maintaining clinical rooms and equipment before, during and after patient care activities, ensuring a clean and safe environment
  • Ensuring emergency medication and resuscitation equipment is adequately stocked and fit for purpose
  • Ensuring proper handling and dispatch of laboratory specimens including blood and urine samples
  • Participating in practice meetings and contributing to our aim for continuous improvement
  • Supporting the training and mentoring of junior staff members and HCAs

Person Specification

Experience
Essential
  • Experience of working within a nursing team in primary care
  • Excellent communication and interpersonal skills, and be confident to deal with patients, families and carers
  • Ability to work independently and as part of a multidisciplinary team, using initiative within the level of competence for the position
  • Commitment to patient confidentiality and data protection
  • Accuracy and a strong desire for attention to detail
  • Commitment to continuing professional development

Desirable
  • Working knowledge of SystmOne clinical system
  • Stable career history showing clear commitment and progression

Qualifications
Essential
  • Registered General Nurse with current NMC registration
  • Qualified & competent in cervical cytology, immunisations, and chronic disease management and phlebotomy

Desirable
  • Independent Prescriber
