Community Rehabilitation Programme Facilitator

Location: Wickford

Salary: £15,600 per annum

**Please be advised this position is 21 hours per week, part-time, the salary advertised reflects the hours**

Are you passionate about supporting people to make lasting, positive changes in their lives? We are looking for a compassionate and motivated Recovery Facilitator to join our team at the Essex Community Rehabilitation Centre, delivering The Forward Trust's SHARP Programme.

The SHARP Programme is a structured community day rehabilitation programme based on the 12-Step approach, supporting adults aged 18+ affected by drug and alcohol use to achieve sustainable recovery.

As a Recovery Facilitator, you will play a key role in helping individuals transform their lives by managing a caseload of service users throughout their recovery journey. You will:

  • Deliver structured daily group programmes and therapeutic interventions based on the SHARP model.
  • Complete comprehensive assessments and develop person-centred SMART care plans.
  • Support service users through the admissions process, working closely with Recovery Workers across the Essex region.
  • Build positive, professional relationships that encourage engagement, motivation and long-term recovery.
  • Maintain accurate records using client information management systems.
  • Work collaboratively with colleagues and partner agencies to ensure the best outcomes for service users.

This is a varied and rewarding role where no two days are the same. Flexibility is important, as occasional evening or weekend working may be required, along with travel to other local services to support continuity of care.

About You

We're looking for someone who is enthusiastic, empathetic and committed to supporting people to achieve meaningful recovery. You'll be confident working both independently and as part of a multidisciplinary team, with excellent communication and relationship-building skills.

You'll ideally have:

  • Experience carrying out assessments and developing SMART care plans.
  • A positive, solution-focused approach with strong problem-solving skills.
  • Excellent communication, negotiation and influencing skills.
  • The ability to engage and motivate people from diverse backgrounds.
  • A good understanding of equality, diversity and inclusion, with the ability to provide person-centred support.
  • Experience using Microsoft Office and video conferencing platforms, with the confidence to support service users with digital literacy where needed.
  • An understanding of the 12-Step recovery model.
  • Knowledge and experience of psychosocial interventions, including facilitating structured recovery programmes.
  • Experience using client information management systems to maintain accurate records.
