We are looking for an ANP who: * Is an experienced Advanced Nurse Practitioner with a proven background in Primary Care. * Holds an independent prescribing qualification (essential). * Is confident in assessing, diagnosing, treating, and managing patients with a wide range of clinical presentations. * Demonstrates excellent clinical judgement, communication, and decision‑making skills. * Enjoys working collaboratively within a supportive multidisciplinary team while also being able to work independently. * Is committed to delivering safe, compassionate, and high‑quality patient care. Main duties of the job The Advanced Nurse Practitioner will be expected to undertake all duties and responsibilities associated with an Advanced Nurse Practitioner working within Primary Care, including (but not limited to): Work autonomously as an advanced practitioner with an extended scope of practice, contributing to the delivery of comprehensive Primary Care services. Conduct face‑to‑face consultations, telephone assessments, and occasional home visits, providing accessible and responsive care to our patients. Perform clinical assessments, take patient histories, formulate diagnoses, and develop evidence‑based treatment and management plans. Prescribe independently within the scope of professional practice, ensuring safe and effective patient care. Maintain accurate, detailed, and contemporaneous clinical records that clearly document assessment, decision‑making, and interventions. Collaborate closely with our multi‑disciplinary team, sharing clinical expertise and contributing to a culture of continuous learning and excellence. Ensure all clinical practice aligns with the latest evidence‑based guidelines, supporting a modern, prudent, and patient‑centred approach to Primary Care. About us We are a well‑established two‑site practice serving over 13,000 patients across a rural community, with a dedicated triage hub at the heart of our service. Our multidisciplinary team includes GPs, ANPs, nurses, pharmacy, wellbeing and mental health professionals, supported by an experienced admin team all working together to deliver high‑quality patient care. Collaboration is central to our approach. We work closely within the Practice and with external partners to ensure seamless, responsive healthcare. Job responsibilities Please refer to ANP - JD and Person Specification attached for full job description. Clinical responsibilities * Diagnosing and treating patients of all ages presenting with minor illness. * Ensuring clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations. * Provide first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of advanced skills in history taking, physical examination, problem solving and clinical decision making to establish a diagnosis and management plan. * Instigate necessary invasive and non-invasive diagnostic tests and investigations and interpret findings and action within own competence. * Prescribe and review medication for therapeutic effectiveness, in a safe, effective and appropriate manner as defined by current legislative frameworks and local guidelines, and within scope of practice. * Provide safe, evidence-based individualised care to the practice population. * Refer patients directly to external services/agencies as appropriate. * Work with the multi-disciplinary team within the practice and contribute to practice targets (local and national) with disease management agenda. E.g. QoF. * Undertake scheduled home visits for patients unable to attend the surgery as agreed with the Duty GP and within scope of practice. Administration and professional responsibilities * Ensure accurate and legible notes of all consultations and treatments are recorded in the patients records. * Dealing appropriately with results and clinical letters for patients under your care. * Ensure accurate completion of all necessary documentation associated with patient health care and registration. * Attend and participate in practice meetings as required.
