Job descriptionWe are a small friendly, supportive PCN, seeking to appoint an AdvanceNurse Practitioner to join our team. We are looking for applicants who are energetic, enthusiastic andfocused on delivering a high standard of care to our patients. You should beproficient in all aspects of general practice nurse duties; management ofLong-Term Conditions is advantageous. We care for approximately 30K patients using SystmOne, comprisingof four GP Practices Grafton Road Surgery, Hobs Moat Medical Centre, RichmondMedical Centre and St Margarets Medical Practice. Main duties of the job The successful Advanced Nurse Practitioner willwork as an autonomous practitioner, diagnosing, managing, prescribing anddischarging patients with illness or injury across all age groups includingyoung children. This may be by face-to-face consultation, telephone or videoconsultation. Registered Adult Nurse registered with the Nursing &Midwifery Council (NMC), Paramedic registered with the Health Care ProfessionsCouncil (HCPC) or Pharmacist registered with the General Pharmaceutical Council(GPhC) Hold an Advanced Nurse Practitioner qualification/Independentprescriber qualification Can show evidence of using autonomous physical examinationand assessment skills in practice Can demonstrate a broad and sound knowledge of complex andcommon conditions in primary care in both adults and children Knowledge and experience of QOF desirable About us To work as part of the primary healthcare team, providing medicalservices to the practices patients; including clinical assessment, diagnosis, treatment,preventive care, screening and patient education. This role will involve working alongside a multidisciplinaryteam, including GPs, Nurses, Clinical Pharmacists, Physicians Associate and HCAsto provide care and treatment to patients with either long term conditions orundiagnosed undifferentiated diagnoses. Job responsibilities Key Responsibilities: * 15-minute consultations, catch up/admin slots * Able to manage acute presentations * Able to manage chronic conditions * Able to order appropriate investigations (bloods/radiology). * To prescribe safe, effective and appropriate medication as defined by current legislative framework * Utilise referral pathways to secondary care services, community services, locality services and organise emergency admissions. * Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care * Able to file pathology results * Able to see all age groups * No restrictions on prescribing. * Able to see pregnant women in regards non pregnancy related issues. * Able to deal with mental health presentations Please see job description document attached to this advert Person Specification Experience * Minimum of 3 years post registration * Experience * Evidence of appropriate continuing * professional development activity to * maintain up-to-date knowledge and ongoing competence in all aspects of the ANP role * Experience in management long term conditions e.g. asthma, COPD, diabetes, CHD * Evidence of appropriate continuing * professional development activity to * maintain up-to-date knowledge and ongoing competence in all aspects of the ANP role * Evidence of working autonomously and as part of a team * Proven ability to evaluate the safety and effectiveness of their own clinical * practice Qualifications * Registered General Nurse (currently * registered with the Nursing & Midwifery Council) * Recognised ANP qualification at * minimum of degree level or equivalent * RN Advanced Nurse / clinical Practitioner programme accreditation Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
