Location: Saltash & Torpoint INT / Liskeard & Looe INT Salary:£55,000 per annum Contract: Fixed-Term for 2 Years Reports to: PCN Manager Shape the Future of Neighbourhood Health in Cornwall Kernow Health CIC is seeking two ambitious and experienced NeighbourhoodHealth Programme Managers to help lead the development and delivery of Integrated Neighbourhood Teams (INTs) across East Cornwall. This is an exciting opportunity to play a key role in transforming how health, care and community services work together to improve outcomes for local people. Working alongside clinical leaders, partner organisations and communities, you will support the delivery of neighbourhood priorities, coordinate complex programmes of work and help embed the Integrated Care System (ICS) neighbourhood model across Cornwall and the Isles of Scilly. Main duties of the job As Neighbourhood Health Programme Manager, you will work closely with the Neighbourhood Clinical Lead, PCN Manager, Place Director, Development Manager for the Integrated Care Area and a wide range of partner organisations to coordinate neighbourhood delivery plans, support transformational workstreams and ensure robust governance, reporting and assurance processes are in place. Initially, the role will focus on establishing strong neighbourhood working arrangements, supporting Neighbourhood Steering Groups and developing realistic, deliverable plans. As neighbourhood arrangements mature, opportunities may arise to take on greater operational oversight within agreed governance arrangements. Job responsibilities The Neighbourhood Health Programme Manager will support the Neighbourhood Leadership Team by coordinating delivery of agreed neighbourhood priorities, maintaining the neighbourhood delivery plan, supporting workstreams, producing high-quality reports, and ensuring risks, decisions and resource issues are clearly escalated through agreed governance routes. Working closely with the Clinical Lead, PCN Manager, Place Director, Development Manager for the Integrated Care Area and partner organisations, the postholder will help embed effective neighbourhood structures, delivery mechanisms and assurance processes. The postholder will also contribute to the wider network of Neighbourhood Programme Managers across Cornwall and the Isles of Scilly, supporting shared learning, consistency of approach and development of the ICs neighbourhood model. This is a senior programme delivery and coordination role. It is initially focused on establishing effective neighbourhood working arrangements, supporting the Neighbourhood Steering Group, defining priorities, developing a realistic delivery plan and coordinating agreed workstreams. As integrated neighbourhood arrangements mature, the role may include greater operational oversight, within the limits of agreed governance and delegated authority. Main Duties * Coordinate the monthly Neighbourhood Steering Group / Project Board, including agendas, papers, reports, action tracking and decision logs. * Coordinate neighbourhood workstream meetings and ensure actions, risks, dependencies and decisions are visible and followed through. * Maintain the neighbourhood delivery plan, setting out agreed priorities, milestones, risks, metrics and progress updates. * Produce clear performance, finance, risk and progress reports for the Steering Group and wider governance routes. * Support neighbourhood financial monitoring, working with the PCN Manager, finance colleagues and partners to identify risks, funding requirements and value for money considerations. * Analyse neighbourhood data and produce meaningful insight to support decision-making, using Excel and other analytical or reporting tools as required. * Support development of business cases, funding applications and service improvement proposals, ensuring these are evidence-based and routed through appropriate governance. Key Responsibilities 1. Programme Leadership and Delivery Coordination * Support the development of a high-performing Neighbourhood Steering Group by ensuring clear agendas, papers, actions, decisions and reporting arrangements. * Develop, maintain and review the Neighbourhood Delivery Plan, ensuring priorities, milestones, risks and measures of success are clearly documented. * Act as the programme coordination function for agreed neighbourhood workstreams, helping partners maintain momentum and clarity. * Work with the Integrated Care Area Place Director and Development Manager to ensure neighbourhood plans align with Integrated Care Area priorities. * Build constructive relationships with primary care, community services, local authorities, VCSE partners, acute services, mental health services and communities. * Identify delivery barriers and elevate them with clear options, evidence and recommendations. 2. Service Transformation and Pathway Development * Support mapping of services, assets and gaps to identify opportunities for integration, improvement and better use of existing resources. * Coordinate agreed pathway improvement work to strengthen proactive, preventative and community-based care. * Use Population Health Management information, service data and partner intelligence to support targeted interventions and reduce inequalities. * Support the appropriate use of digital tools, reporting systems and innovative approaches where these improve delivery, access or outcomes. * Support partners to explore opportunities for shared or improved estate usage, where this is relevant to agreed neighbourhood priorities. 3. Performance, Data, Governance and Financial Monitoring * Develop and maintain proportionate performance reporting, using data to track outcomes, quality, access, delivery and impact. * Analyse neighbourhood data and synthesize trends, risks and issues for decision-making. * Produce monthly reports for the Neighbourhood Steering Group, including performance analysis, progress updates, risks and issues, finance monitoring and decisions required. * Maintain a clear risk and issue log, decision log and action tracker for neighbourhood work. * Support financial monitoring and business case development, working within the agreed financial governance framework. * Support workforce planning discussions by coordinating information, identifying implications of proposed changes and ensuring recommendations are taken through appropriate partner governance routes. * Ensure neighbourhood governance arrangements are clear, proportionate and aligned with agreed PCN, partner and system governance requirements. 4. Continuous Improvement and Learning Culture * Apply quality improvement principles to support agreed service improvements and delivery reviews. * Promote peer learning and shared development across neighbourhood and ICs networks. * Use patient, staff, resident and community insight to inform neighbourhood priorities and service improvement work. * Capture and share learning to support the development of the wider ICs neighbourhood model. Decision-Making and Delegated Authority The postholder will make day-to-day programme coordination decisions within the agreed delivery plan and within any delegated limits confirmed by the employer and neighbourhood governance arrangements. The postholder will not independently commit partner organisations to service changes, workforce changes, material expenditure, contractual arrangements or changes in clinical accountability. Decisions requiring partner, financial, workforce, contractual or service-change approval must be escalated through the agreed governance route. The postholder is expected to provide evidence-based recommendations, identify risks and options, and support implementation once decisions have been made by the appropriate governance body. Person Specification Skills and Abilities * Strong programme and project coordination skills; * strong analytical capability, including ability to interpret complex data and turn it into clear insight; * advanced Excel skills, including pivot tables, lookups and dashboard-style reporting; * excellent report-writing and communication skills; * strong relationship-building, facilitation and negotiation skills; * ability to manage competing priorities and work autonomously; * ability to chair or coordinate multi-agency meetings and drive action through others. * Ability to design or maintain digital reporting dashboards, such as Power BI; * skills in service mapping, demand and capacity modelling or data visualisation Experience * service improvement, transformation or change management within health, social care, public sector or VCSE settings; * experience supporting complex multi-agency projects; * experience working with or alongside primary care, community services, local authorities or integrated care models; * experience producing reports, tracking risks/ actions and supporting governance processes; * experience analysing data and presenting insight for decision-making. * Experience working directly with ICSC/ICB structures; * experience in neighbourhood, locality or place-based working; experience supporting budget monitoring, business cases or funding applications; * experience of operational service management within agreed governance arrangements. Other Requirements * Able to travel across Cornwall and Isles of Scilly as required; * flexible approach with ability to work across different organisational settings. * Full driving licence and access to transport, where this is required to fulfil the duties of the role. Qualifications * Degree-level qualification or equivalent experience; * evidence of continuous professional development; * training or experience in project/programme management, change management or service improvement methodologies. * Postgraduate qualification in a relevant field; * formal project/programme management qualification such as PRINCE2, MSP or Agile; * Quality Improvement training. Knowledge * Understanding of population health management, health inequalities, wider determinants of health and community-based models of care; understanding of integrated care, partnership working, governance, risk management and programme assurance. * Knowledge of local health and care services in Cornwall and Isles of Scilly; * understanding of estates, digital or workforce transformation.
