Liquid Personnel is recruiting Social Worker for its clients Community Team in Hounslow.
As a Social Worker in the Adult Community Team you will manage a varied caseload of adults with a range of social care needs. Working closely with individuals families health professionals and community partners you will promote independence wellbeing and informed choice while ensuring statutory responsibilities are met.
You will play a key role in supporting vulnerable adults undertaking assessments coordinating care and safeguarding those at risk.

Manage a caseload of adults with varying levels of need including complex cases depending on experience
