Job description
We are seeking an experienced physiotherapist to join our rapidly developing stroke service in Carmarthenshire, supporting acute rehabilitation and early discharge from hospital. If you have a passion for stroke rehabilitation working across the stroke pathway this could be the opportunity for you. You will be based at Glangwili General Hospital, Carmarthen supporting people who have predominantly experienced a stroke to return home from our acute stroke units. The role will aim to deliver rehabilitation in hospital and at home in the local community to support transition from hospital and return to independence. To deliver a person‑centred rehabilitation and transition pathway the post holder will work alongside the established acute and community stroke teams (in‑reaching and out‑reaching) to ensure good continuity of care. We are advertising for 1 post holder – based in Glangwili General Hospital, Carmarthen. Main duties of the job Successful applicants will have experience of working with people who have experienced stroke and/or other life changing events and enjoy working as part of a multi‑professional team within the acute and community setting. You will work alongside acute and community colleagues and services to create a joined up rehabilitation pathway and positive experience for our population. Whilst the role focusses on stroke, we require therapists who are adaptable to meet needs of patients with multiple and a variety of conditions as well as service priorities. Due to the nature of the role you will need to be able to travel within the County, as well as being flexible to travel across the Health Board on occasions. The post holder will be required to participate in the provision of on call respiratory service and future developments in 7 day working. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Job responsibilities The role requires delivery of rehabilitation in both hospital and community settings, supporting transition from hospital and return to independence. The post holder will work with acute and community stroke teams to ensure a seamless rehabilitation pathway. The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements specified, the Health Board offers a variety of learning options and staff support to help you meet these minimal desirable requirements during the course of your employment with us. Person Specification Qualification and Knowledge * Professional registration recognised by HCPC * Postgraduate evidence of specialist education. * CPD evidence. * IT Skills * Post‑graduate qualification up to post‑grad diploma level in relevant specialty * Clinical Education Training Course. * Member of C.S.P * Active participation of specialist interest group. Experience * Post‑graduate experience in relevant clinical specialty. * Completion of junior competency framework. * Practical experience to a level that would enable participation on the respiratory on call rota. * Therapeutic handling and risk assessments. * Managing a diverse caseload using a clinical reasoning process. * Inter‑professional and team working. * Direct involvement with carers and families. * Evidence of supporting service compliance with clinical governance requirements. * Experience of staff supervision. * Evidence of student supervision. * Negotiation of MDT working practices. * Experience of service development. * Experience of NHS working. Other * Evidence of teaching / In‑Service Training. * Flexible approach to meet the needs of the service. * Prepared to participate in on call and extended service. * Ability to travel between sites and in community in a timely manner.
