We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy Department. This is a Part time role for 22.5 hours a week. The role holder will be required to cover a flexible shift pattern. Duties of this role include: * Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation. * Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations. * Support safe prescribing through reconciliation, clinical screening, and dose adjustment. * Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements. * Promote safe and cost-effective use of medicines by supporting adherence to local and national policies. * Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity * Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance Applicants should meet the following criteria: * A degree in pharmacy MPharm, BSc or BPharm. * GPhC registration as a Pharmacist. * Previous experience in a clinical hospital pharmacy role. * Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation. * Excellent communication and clinical decision‑making skills. * Ability to work autonomously and as part of a multidisciplinary team. * Confident using electronic prescribing and medication administration systems (e.g., EPMA).
