Job description
Care Coordinator – Domiciliary Care Location: Salford, Greater Manchester Salary: Up to £31,000, depending on experience Hours: Monday to Friday, 8:30am–4:30pm On-call: Shared evening and weekend rota with two colleagues We are recruiting for an experienced Care Coordinator to join an established domiciliary care provider supporting adults to live safely and independently in their own homes. This is a busy and varied position requiring someone who can coordinate care packages, manage staff rotas and respond calmly when circumstances change. You will work closely with the Registered Manager, care staff, service users, families and external professionals. Key Responsibilities Create and maintain daily and weekly care rotas. Ensure all care visits are covered safely and efficiently. Match care workers with service users based on skills, location, availability and continuity. Respond to sickness, emergencies, hospital admissions, new referrals and changes to care packages. Ensure care plans, risk assessments and relevant clinical guidance remain accurate and accessible. Support the implementation of new care packages, amendments and discharges. Communicate rota changes clearly to care staff and service users. Provide day-to-day guidance and support to care workers. Maintain accurate records on care management and rostering systems. Assist with audits, spot checks, complaints, safeguarding concerns and incident investigations. Liaise with families, social workers, district nurses, GPs, therapists and commissioning teams. Identify recurring staffing, rota or service-delivery issues and escalate them appropriately. Participate in the shared out-of-hours on-call rota. Essential Requirements Previous experience within domiciliary care or a similar community-based care service. Experience working as a Care Worker, Senior Carer, Team Leader or Care Coordinator. Experience planning rotas, allocating staff or scheduling services in a fast-paced environment. Good understanding of person-centred care, safeguarding and promoting independence. Awareness of CQC Fundamental Standards and domiciliary care requirements. Confident using email, Microsoft Office and care management or rostering systems. Strong organisational and time-management skills. Excellent verbal and written communication. Ability to remain calm, make sensible decisions and manage frequent changes. Flexibility to participate in the evening and weekend on-call rota. Enhanced adult workforce DBS and satisfactory references. Desirable Experience NVQ/QCF Level 3 in Health and Social Care, or currently working towards it. Previous experience in a Care Coordinator or Senior Coordinator position. Experience supporting adults with dementia or complex needs. Knowledge of medication management, mental capacity and community risk assessments. Experience contributing to audits, inspections or quality-improvement plans. Full UK driving licence and access to a vehicle. About You You will be reliable, organised and able to manage several priorities without losing attention to detail. You should be approachable and confident when communicating with care staff, service users and families. Most importantly, you will be committed to providing safe, respectful and person-centred care while supporting people to remain independent in their own homes.
