Job description

Help people rebuild their lives. Be part of the solution to homelessness.

We are proud to deliver our Single Homelessness Accommodation Programme (SHAP), a specialist supported housing service providing ten self-contained one-bedroom properties for single adults and couples, including accessible accommodation for people with additional mobility needs.

Our service offers safe, stable accommodation alongside intensive, person-centred support for people who are homeless or at risk of homelessness. Many of the individuals we support have experienced long-term rough sleeping, trauma, poor mental health, substance misuse, or other complex needs.

Using Trauma-Informed and Psychologically Informed Environment (PIE) approaches, we work alongside each person to build confidence, develop independence, and help them move towards sustainable, long-term housing.

As a Support Worker, you will play a key role in preventing homelessness and supporting people to achieve lasting positive change. You'll build trusted relationships with customers, providing practical and emotional support that enables them to overcome barriers, maintain accommodation, and work towards independent living.

This is a varied and rewarding role where no two days are the same. You'll work closely with internal teams and partner agencies to deliver coordinated, person-centred support tailored to each individual's goals and circumstances.

Key Responsibilities

  • Provide high-quality, person-centred support to adults experiencing homelessness or at risk of homelessness, including those with complex needs.
  • Manage a caseload of customers, completing assessments and developing personalised support plans focused on achieving sustainable outcomes.
  • Complete and maintain support plans, risk assessments, safeguarding records, and case notes using the INFORM system.
  • Support customers to access and sustain suitable accommodation, including social housing applications, supported housing referrals, and private rented options where appropriate.
  • Help customers maximise their income by supporting benefit claims, budgeting, debt management, and financial inclusion.
  • Work collaboratively with health professionals, social care, probation, substance misuse services, mental health teams, and other partner agencies to ensure coordinated support.
  • Advocate on behalf of customers, assisting with appointments, applications, grant funding, and accessing essential services.
  • Identify safeguarding concerns and make referrals to Adult Social Care and other appropriate agencies where required.
  • Encourage customers to develop independent living skills, improve wellbeing, build social connections, and work towards personal goals.

What We're Looking For

We're looking for someone who is compassionate, resilient, and committed to making a positive difference in people's lives. You'll be able to build trusting relationships while maintaining professional boundaries and be confident supporting people with a range of complex needs.

Experience in homelessness, housing, supported accommodation, mental health, substance misuse, or related support services is desirable, but above all we're looking for someone who shares our values and believes everyone deserves the opportunity to rebuild their life.

Why Join Us?

At SHAP, we believe that homelessness is not just about housing—it's about helping people regain stability, confidence, and hope for the future.

By joining our team, you'll become part of a service that changes lives every day through compassion, partnership, and person-centred support.

If you're passionate about preventing homelessness and empowering people to achieve independence, we'd love to hear from you.
