Overview LAP Triage Admin role based in Cheadle, Stoke on Trent. Closing date: 23 July 2026. Main duties As administrator for this service, you will be part of the LAP Call Handling Team and act as the first point of contact for messages, queries and referrals to the Community Nursing Services in North Staffordshire. You will provide high-quality customer service and direct callers to the most appropriate service for ongoing care, support and advice. You will identify and forward urgent calls and enquiries to the triage nursing team when they cannot be dealt with at source, requiring nursing expertise. You will communicate regularly with patients and families, including those experiencing distress or receiving difficult news. You will use varying IT systems to support the team and ensure efficient operations. * Action appropriate referrals, ensuring demographics are correct and plan outcomes within the agreed scope. Return any inappropriate referrals to the sender for further information. * Ensure a minimum standard of information is recorded for all referrals and messages; contact the referrer or patient if the call is not taken directly by the call handler. * Process referrals on electronic clinical systems following agreed protocols; ensure all new referrals are sent to the clinical triage team in a timely way. * Demonstrate activities to new starters, support induction for new staff and maintain an up-to-date new starter handbook. * Participate in team meetings and huddles, taking minutes and completing actions as appropriate. * Work with the PALS service to capture service user feedback and drive service improvements.
