Job IntroductionAs a Band 6 Occupational Therapist, you will join our Early Supported Discharge Service based on Sapphire Ward at Gravesham Community Hospital. This is a rewarding opportunity to support patients recovering from stroke and neurological conditions as they transition from hospital to home. You will play a key role in delivering high quality, patient centered rehabilitation, helping people regain independence and confidence in their daily lives. This is a varied role that combines inpatient work with community visits, giving you a unique opportunity to follow patients through their recovery journey. You will be part of a dynamic and supportive multidisciplinary team who are passionate about delivering excellent care and continually improving the service.
Package DescriptionYou will be joining a collaborative and forward thinking therapy team who are committed to delivering high quality stroke and neurological rehabilitation. The team provides a supportive environment where development is encouraged, ideas are valued and colleagues work closely together to achieve the best outcomes for patients.
