The Service & Team:

This is an exciting opportunity to join the Countywide Access Team within Adult Social Care. The Access Team is the front door into Adult Social Care for adults carers families professionals and partner agencies. The team receives referrals information requests and concerns through a range of routes and plays a key role in helping people access the right advice support and next steps at the earliest opportunity.

The service is focused on promoting independence preventing reducing and delaying the need for ongoing care and support and ensuring that people are supported in a timely proportionate and person-centred way. You will be joining a busy supportive and committed team working closely with colleagues across Adult Social Care health partners community services and the voluntary sector.

The Role:

As a Senior Access Assessor you will support the day-to-day management of the Access Team and help ensure effective processes are in place to respond to contacts identify presenting needs assess risk and determine the most appropriate next steps.

You will provide guidance mentoring and support to Access Assessors including new and less experienced staff. You will help oversee the triage folder support sufficient cover during core hours and provide advice in relation to risk safeguarding concerns self-neglect and complex presenting situations.

The role will also involve gathering information liaising with professionals and partner organisations supporting strengths-based conversations providing information and advice and ensuring records are accurate and proportionate. You will play an important role in supporting quality consistency and timely decision-making across the front door of Adult Social Care.

This is a public/customer-facing role where the statutory English language requirement for public sector workers applies.

This position will be subject to an enhanced criminal record disclosure check.

Working Pattern:

This is a full-time role working within the Countywide Access Team. The working pattern will be in line with service need and will usually be within core business hours.

Cornwall Council supports flexible working where this meets the needs of the service. This may include hybrid working arrangements with a balance of office-based and remote working subject to operational requirements. Flexible working options can be discussed as part of the recruitment process.

What youll need to succeed:

We are looking for someone who is confident organised and able to support others in a busy front door service. You will need experience of working in a customer-focused environment and experience or understanding of working with people who have care and support needs.

You will have strong communication skills a good telephone manner and the ability to listen carefully ask appropriate questions and gather relevant information. You will be able to evaluate information assess risk make timely and proportionate decisions and support others to identify the best next steps.

You will need to be self-motivated able to prioritise your work remain calm under pressure and maintain a professional focus in a busy and demanding environment. You will also need good IT and recording skills and an understanding of relevant legislation including the Care Act 2014 Mental Capacity Act 2005 Human Rights Act and Data Protection.

An understanding of adult social care safeguarding inter-agency working and community-based support will be important. Experience or an interest in developing management skills including mentoring or supporting others would also be beneficial.
