Advanced Practitioner

Job summary
Phoenix Family Practice is looking for a prescribing Advanced Nurse or Paramedic to help with urgent and acute attendances on the day, assist with emergency presentations and assist with compliance.

Our triaging hub will decide on those acute presentations that require a full workup, including ECGs, bloods, swabs and a practitioner that can put the whole picture together.

Main duties of the job

• To provide capacity on the day, seeing undifferentiated and acute presentations.
• Assessing patients, taking note of symptoms and using both practical assessment skills and consultation to develop a differential diagnosis
• To undertake a variety of investigations, including ECGs, BPs, bloods and follow up on the results to ensure patients are treated effectively.
• To provide a treatment, immediately when required for example with nebuliser, or as part of a longer term treatment plan by prescribing phophylatic or acute medication.
• Overseeing an aspect of our urgent care infrastructure, taking ownership of basic life saving equipment such as defibrillators, oxygen machines and ensuring that emergency drugs are maintained as appropriate.

About us
Phoenix Family Practice is a large practice service 25,000 patients of Weybridge and parts of Addlestone, Byfleet, Hersham and Walton.

We are based on the Weybridge Hospital site in central Weybridge. In April 2027 we will be moving into a brand new building on the site.

We are a training and teaching practice, with strong links to Imperial College London and St George's Medical schools. We also have GP Trainers and Resident Doctors from the Kent, Surrey and Sussex Deanery working with us.

We run a GP-led total triage system, where appointment allocation is based on clinical requirement.

Job responsibilities
The Advanced Clinical Practitioner (ACP) will work as an autonomous clinician within the multidisciplinary general practice team, providing high-quality, patient-centred care to patients presenting with both acute and long-term health conditions. Working within their agreed scope of practice, the post holder will assess, diagnose, investigate, treat and appropriately refer patients, using advanced clinical decision-making skills to deliver safe and effective care.

The ACP will manage a varied clinical caseload, including same-day urgent presentations, minor illness and chronic disease managements. They will contribute to improving patient access, reducing GP workload and supporting the delivery of high-quality primary care services.

In addition to their clinical responsibilities, the ACP will contribute to service development, clinical governance and quality improvement initiatives, supporting the practice in delivering safe, effective and responsive healthcare. The post holder will work collaboratively with GPs, nurses, pharmacists and the wider multidisciplinary team, providing clinical leadership, sharing knowledge and supporting the development of colleagues where appropriate.

The post holder will maintain professional registration with the NMC or HCPC, practise in accordance with the relevant professional code of conduct, and ensure their knowledge and skills remain up to date through continuing professional development. They will work within national guidance, local policies and the practice's clinical governance framework, recognising the limits of their competence and seeking advice or supervision when required.

The role may also include participation in practice meetings, audit, education, service improvement projects and the supervision of students or other healthcare professionals, depending on experience and the needs of the practice.
