We are offering an exciting opportunity to join our supportive and friendly Community Nursing Ambulatory clinic team based across Cannock and surrounds as a Band 5 NMC Registered Nurse.

We would love to hear from you if you are passionate about being part of a team delivering the highest standards of wound care, tissue viability and catheter care to patients within a clinical setting.

Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Working within our Community Nursing Team you will support, assess, and treat patients with a variety of health needs, delivering high quality, evidence-based practice.You will holistically support patients to manage their long term conditions and care needs within the community and support patients with acute care needs to receive treatment (such as IV antibiotics) without an avoidable hospital admission.

We offer bespoke training, support and personalised ongoing professional development into a variety of career pathways.

You will be required to travel independently across a large geographical area.

Main duties of the job
  • Assess new service user referrals and care/support needs.
  • Provide care directly to patients on the caseload with a range of nursing needs (complex wound care, pelvic health, medication administration).
  • Promote independence and support patient centred care planning.
  • Work autonomously, in line with your scope of practice, competence and policies.
  • Handover patient information to the wider team, document patient information effectively.
  • Escalation of patients' needs as appropriate.
