Job SummaryBe the Occupational Therapist Patients Need Delivering Proactive Frailty and Independence Support in Primary Care. Are you passionate about helping people remain independent, active and well within their communities? We are seeking an experienced Occupational Therapist to join our multidisciplinary primary care team, delivering proactive, preventative and personalised care to patients living with frailty, long-term conditions and complex needs. This is a 12 month fixed-term contract initially and an exciting opportunity to shape and deliver a developing Occupational Therapy service embedded within Primary Care Networks (PCNs). The successful candidate will be supporting patients earlier in their journey and helping prevent deterioration, loss of independence and avoidable escalation of care. We are happy to consider applicants on a full and part-time basis. The post holder will work across the PCN footprint in the areas of Hornchurch, Upminster, Cranham & Romford, supporting all participating practices and contributing to neighbourhood models of care. Working as part of the multidisciplinary team, the Occupational Therapist will deliver proactive, preventative and personalised care. You will deliver occupational therapy clinics, support multidisciplinary care planning, contribute to group consultations and help improve outcomes for patients whose physical, cognitive, social or environmental needs impact their ability to undertake everyday activities.
Main duties of the jobThe Occupational Therapist will provide specialist occupational therapy assessment and intervention across the Primary Care Network, supporting patients experiencing:

Job responsibilitiesClinical ResponsibilitiesProvide specialist occupational therapy assessment and intervention including:
- Comprehensive occupational therapy assessments within primary care.
- Functional assessment of activities of daily living and instrumental activities of daily living.

The post holder will:
- Participate in MDT meetings and case discussions.
- Support proactive case management of complex patients.
- Contribute to anticipatory care planning.
- Provide specialist occupational therapy advice to the wider MDT.
- Facilitate appropriate onward referrals into community services.
Frailty and Proactive Care- Support proactive frailty identification and management.
- Contribute to frailty MDTs and neighbourhood teams.

Health Inequalities and Personalised Care- Support patients experiencing barriers to accessing healthcare.

Service Development and Quality Improvement- Support development of occupational therapy pathways within primary care.

Person SpecificationQualificationsEssential- BSc or MSc in Occupational Therapy.
- Current HCPC registration.
