Join HCRG Care Group as a Healthy Family Practitioner based in Skelmersdale and play a vital role in supporting children, young people, and families across Lancashire.

Working as part of the Young People & Healthy Families Service, you’ll collaborate closely with Health Visitors, School Nurses, and wider multidisciplinary teams to deliver high-quality, compassionate care. This is a rewarding community-based role where you’ll empower families, promote child development, and help improve long-term health outcomes.

This is a part-time fixed-term opportunity working 30 hours per week with a planned start date of September 2026.

What you'll be doing:

In this varied and impactful position, you will:

  • Deliver key elements of the Healthy Child Programme, including developmental reviews (e.g. 1-year and 2–2.5-year checks)
  • Support children and families with targeted interventions such as behaviour management and infant feeding
  • Run community clinics and Healthy Family drop-in sessions
  • Work directly with families in community settings, including home visits
  • Build strong relationships with nurseries and local services
  • Identify safeguarding concerns and escalate appropriately
  • Encourage service uptake and act as an advocate for families’ needs and voices
  • Maintain accurate records and contribute to service improvement

Essential Qualifications

  • NNEB qualification or equivalent (e.g. BTEC National Diploma in Nursery Nursing, NVQ Level 3 Childcare & Education, Foundation Degree in Early Years/Education)

Essential Experience & Skills

  • Experience in health promotion or public health activities
  • Team working experience
  • Awareness of safeguarding and child protection
  • Strong understanding of child development and parenting
  • Flexible approach to meet service needs
  • Ability to work independently within a multidisciplinary team
  • Accurate record keeping and ability to maintain high standards of documentation
  • Ability to contribute to audit and research
  • Competence in creating and implementing family‑centred action plans
  • Strong written and verbal communication skills
  • IT literacy, including local and web‑based systems
  • Commitment to continuous learning and development
  • Understanding of clinical governance
  • Full UK driving licence and access to a vehicle
