At IC24, we’re looking for an Advanced Clinical Practitioner (ACP) to join our East Sussex Home Visiting Service. This is an opportunity to work at the forefront of community-based care, delivering high-quality clinical assessments and treatment to patients in their own homes while supporting wider urgent and primary care services. As an experienced clinician, youll work autonomously within your professional scope of practice, providing assessment, diagnosis, treatment and prescribing for patients presenting with acute, urgent and undifferentiated conditions. Youll play a key role in helping patients receive the right care in the most appropriate setting, reducing unnecessary hospital admissions and improving patient outcomes. What Youll Focus On * Delivering advanced clinical care in the community * Making safe, evidence-based decisions * Independent prescribing * Supporting continuity of care * Contributing to service improvement * Developing clinical excellence This role forms part of our East Sussex Home Visiting Service, supporting patients across the community. Youll work as part of a highly skilled multidisciplinary team delivering responsive care where it is needed most, helping patients avoid unnecessary hospital attendance wherever appropriate. Youre Likely Someone Who * Enjoys working autonomously while remaining part of a wider multidisciplinary team * Is confident assessing and managing urgent and undifferentiated presentations * Has strong clinical reasoning and risk management skills * Communicates effectively with patients, carers and healthcare professionals * Enjoys delivering care within patients homes and community settings To Be Successful In This Role, Youll Need * Professional registration with the NMC, HCPC or GPhC * Independent Prescriber qualification * Advanced assessment and minor illness qualification (Level 6 or 7) * Proven experience of autonomous clinical practice * Strong clinical decision-making skills and ability to manage complex presentations * Excellent communication and interpersonal skills * Commitment to continuous professional development and reflective practice * Evidence of service improvement and innovation Who are we?
