Job description
We are excited to recruit a Clinical Pharmacist to join ourexpanding multidisciplinary team as part of a new in-house pharmacy service atHollyns Health & Wellbeing. This is an excellent opportunity to play a keyrole in developing and delivering an innovative, patient-centred pharmacyservice that supports high-quality primary care. Working closely with GPs, nurses, and other healthcareprofessionals, you will provide expert clinical pharmacy input to optimisemedicines use, improve patient outcomes, and enhance prescribing quality andsafety. You will undertake structured medication reviews, support themanagement of long-term conditions, manage medication-related queries, andcontribute to the safe and effective use of medicines across the practice. The postholder will contribute to quality improvementprogrammes, clinical audits, medicines optimisation initiatives, and servicedevelopment, ensuring care is delivered in line with national guidance andlocal priorities. You will also support the implementation of evidence-basedprescribing and identify opportunities to improve patient safety andefficiency. This is an exciting opportunity for a proactive andmotivated Clinical Pharmacist who is passionate about delivering high-qualitypatient care and helping to establish a sustainable and forward-thinkingin-house pharmacy service within General Practice Main duties of the job Contribute to thedaily work of our in-house pharmacy service, working alongside an AdvancedPractice Pharmacist; supported daily by the medicines management lead. To work closely withGPs, Clinicians and other practice staff and members of the primary healthcareteam in developing your own skills to manage patient care and support highquality, safe, cost effective, rational, and evidence-based prescribing. Demonstrate safe,clinical decision-making and expert care, including assessment and diagnosticskills, for patients within the general practice. The post holder willdemonstrate critical thinking in the clinical decision-making process. They will workcollaboratively with the general practice team to meet the needs of patients,supporting the delivery of policy and procedures. About us Hollyns Health and Wellbeing is a proactive practice with two sites, who offer additional services to the practice population, such as warfarin, pain management, sexual health, Level 2 diabetic clinics and minor surgery. Teamwork is integral in ensuring the service is run as effectively and efficiently for the patients as possible. The successful candidate will be able to continue their service benefits if coming from the NHS in relation to the NHS Pension Scheme and annual leave entitlement. The Surgery has * 12,300 patients approx. * GPs, Nurses, Health Care Assistants, Pharmacists, Advanced Clinical Practitioners, Advanced Nurse Practitioners, Womens Newborn & Infant Practitioner and Administration staff * Chronic disease clinics for diabetes, asthma and COPD * Rated Good by CQC * Active GP and Nursing training and teaching practice * SystmOne clinical system * High QOF achievement * Part of a Primary Care Network with access to additional pharmacists, social prescribers, physiotherapy Job responsibilities Key Duties & Responsibilities Medication reviews, computer based or telephone calls formore complex patients, first source of information for patients aboutmedication. Medication and treatment reconciliation systems forpatients transferred out of and back into the practice, assess unplannedadmissions and systems to prevent further unplanned admissions. Practice formularies maintain. QOF ensure data connectivity and proactive organisation todeliver maximum QOF for the practice. Regular QOF audits. Improve riskstratification of pathways and improve service delivery. Medicines optimisation, support joint goal setting withpatients and appropriate treatment selection to support better medicinesexperience. Reducing waste and improving outcomes. Follow up identified patientgroups targeting better concordance leading to improved outcomes. Monitoring of long-term meds e.g. lithium,amiodarone, sharedcare medications and blood results Monitoring and managing repeat prescription wastageincluding patient adherence. Work with clinical team members on long-term conditions(Diabetes, Asthma, COPD, Depression, Rheumatoid Arthritis, Hypertension,Arthritis, Pain, Heart Failure, Shared care drug monitoring) Manage patients with more complex long-term conditions suchas difficult hypertension, compliance with lipid-lowering therapy. Assess and issue day to day acute medication requests asappropriate. Undertake repeat medication reviews and reauthorisation asappropriate. Review and load repeat medication for new patientregistrations. Identify and support patients where concordance issues. Liaise with community pharmacists to solve common day to dayprescribing problems like preparation availability/alternatives etc. Liaise with community pharmacists for the setting up,monitoring, and updating of all patients on NOMAD trays. Liaise with community nurses and prepare MARS sheets asneeded and support practice anticipatory prescribing process in liaison withcommunity nurses and Community Pharmacy Liaising with care homes and community nurses re dressings Responsibility for lithium and antipsychotic drug monitoringand review of blood results Review the prescribing for patients with multi-morbidity andpolypharmacy. Report and follow up patient drug safety/adverse reactions. Support patient self-care and medicines management andconcordance through education. Support staff education and development. Support development of local services to improve workflowwithin the practice. Work with clinical team members on long-term conditions(Diabetes, Asthma, COPD, Depression, Heart failure drug titration Work with team to implement and maintain Practice formularyin longer term. Development: support development of local services toimprove workflow within the practice Service advancement; deliver specific disease managementclinic through personal development and independent prescribing. Person Specification Experience * Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice. * Recognises priorities when problem-solving and identifies deviations from normal pattern and can refer to seniors or GPs when appropriate. * Able to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct. * Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines. * Ability to promote best practice regarding all pharmaceutical matters.
