Job Overview
This is an exciting opportunity for a pharmacist to take a leading role in the management of long-term conditions, helping to improve patient outcomes while working closely with GPs and the wider practice team. The successful applicant will lead and support the management of patients with long-term conditions including:
Conduct structured medication reviews and optimise prescribing. Monitor, review, and manage patients in accordance with local and national guidelines. Contribute to quality improvement initiatives and chronic disease recall systems. Develop personalised care plans and support patient self-management. Work collaboratively with GPs and the wider multidisciplinary team to deliver high quality, evidence-based care. Full clinical support will be provided to interpret clinical investigations and laboratory results to inform treatment decisions.
