Job description
Clinical PharmacistCircle Reading Hospital 37.5 Hours & Permanent Salary: Up to 42,000.00 per year (depending on level of experience, training and qualification) Circle Reading Hospital is part of Circle Health Group, Britains leading provider of independent healthcare with a nationwide network of hospitals & clinics, performing more complex surgery than any other private healthcare provider in the country. We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy Department. This is a full time role for 37.5 hours a week. The role holder will be required to cover a flexible shift pattern. Duties * Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation. * Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations. * Support safe prescribing through reconciliation, clinical screening, and dose adjustment. * Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements. * Promote safe and cost-effective use of medicines by supporting adherence to local and national policies. * Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity. * Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance. Qualifications * A degree in pharmacy MPharm, BSc or BPharm. * GPhC registration as a Pharmacist. * Previous experience in a clinical hospital pharmacy role. * At least 2 years of post qualification experience. * Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation. * Excellent communication and clinical decision-making skills. * Ability to work autonomously and as part of a multidisciplinary team. * Confident using electronic prescribing and medication administration systems (e.g., EPMA).
