Job descriptionClinical Fellow in Gender Dysphoria & Sexual Health The post is for a 12‑month Clinical Fellow based at TransPlus, 56 Dean Street, Soho, within the Gender Medicine team of Chelsea and Westminster Hospital NHS Foundation Trust. The successful candidate will work 40 hours per week delivering patient‑centred care to individuals presenting with gender dysphoria in a sexual health setting, including assessment, treatment planning, hormone therapy management, and sexual health services. The role also involves multidisciplinary liaison, clinical governance, and service development activities. Closing date: 15 June 2026 Responsibilities * Assessment o Undertake comprehensive, holistic initial assessments of patients presenting with gender dysphoria, including medical, psychological, and social factors. * Treatment planning and ongoing care o Develop, implement, and regularly review individualised care plans in collaboration with patients, ensuring shared decision‑making. o Monitor patient progress, treatment outcomes, and wellbeing, adapting management plans as clinical needs change. o Provide continuity of care across assessment, treatment, and follow‑up stages. * Hormone therapy management o Prescribe and monitor gender‑affirming hormone therapy in line with national guidance and local protocols. o Interpret laboratory results and adjust treatment regimens safely and appropriately. o Manage complications or side effects related to hormone therapy and escalated complex cases when required. * Provide comprehensive sexual health services including risk assessment, screening, diagnosis, and management of sexually transmitted infections (STIs). * Deliver health promotion and prevention advice tailored to trans and non‑binary individuals, including HIV prevention and treatment where appropriate. * Multidisciplinary working and liaison o Work collaboratively with consultants, nursing staff, psychologists, GPs, and allied health professionals to ensure integrated care delivery. o Provide specialist advice and guidance to primary care and other healthcare professionals managing TransPlus patients. o Contribute to case discussions, MDT meetings, and joint care planning across services. * Clinical governance and service development o Contribute to the implementation, evaluation, and continuous improvement of clinical pathways and protocols. o Participate in audit, quality improvement initiatives, and research activities to enhance service delivery and patient outcomes. o Ensure compliance with Trust policies, national standards, and clinical governance frameworks. * Safeguarding and holistic care o Recognise and respond appropriately to safeguarding concerns, including mental health, vulnerability, and social risk factors. o Promote a holistic approach addressing bio‑psycho‑social aspects of care, including signposting to relevant support services. Education and Qualifications * Relevant medical degree and eligibility to work in the UK. * Full Registration with the GMC. * MRCP or MRCOG. * DFRSH (DFFP). Experience * Considerable experience in a medical specialty or Accident & Emergency. * SHO level experience in GUM or HIV medicine. * Experience of audit and clinical governance. * Interest and experience in family planning. Skills and Knowledge * Appropriate clinical and technical skills and abilities. * Computer skills and IT literacy. * Non‑judgmental attitude. * Able to work within a multidisciplinary team. * Knowledge of clinical governance. * Empathy and cultural sensitivity towards LGBT+ people. * Ability to perform speculum examination. * Experience of teaching medical trainees and other professionals. * Experience of giving presentations. * Involvement/interest in relevant research. * Demonstrable evidence of understanding and upholding trust values of respect, kindness, excellence, and safety. * Ability to cope with stressful situations, undertake responsibility, and make effective decisions. * Excellent team player. * Ability to use common sense and maintain confidentiality. * Ability to organise own learning time. * Clear reasons for applying for the post that align with career plans. * Demonstrable use of initiative. Employment Conditions Employment is subject to a six‑month probation period. Some roles may require weekend shifts across multiple sites.
