Job description

Liquid Personnel is recruiting an experienced Social Worker for its clients Adults Learning Disabilities Team based in Somerset. You will support adults with learning disabilities through person-centred assessments care planning and safeguarding promoting independence and positive outcomes.

Working as part of a multi-disciplinary team you will collaborate with partner agencies to deliver effective strengths-based interventions in line with statutory duties.

What will your responsibilities be

  • Undertake Care Act assessments reviews and risk assessments for adults with learning disabilities and their carers
  • Manage a caseload of varying complexity responding to referrals and prioritising need effectively
  • Develop and review person-centred care and support plans promoting independence and wellbeing
  • Carry out safeguarding enquiries and manage risk in line with statutory duties
  • Support individuals in crisis applying appropriate legislation to achieve timely outcomes
  • Apply the Mental Capacity Act including Best Interests decision-making where required
  • Assess carers needs and implement appropriate support plans
  • Prepare clear accurate reports and maintain up-to-date case records
  • Work collaboratively with families advocates and multi-agency partners
  • Facilitate and coordinate care packages with providers across statutory and independent sectors
  • Participate in team duty rota and contribute to service delivery

Qualifications & Experience

  • Degree in Social Work (BA BSc or MA) or an equivalent recognised qualification
  • Registered with Social Work England
  • Relevant experience within Adult Social Care ideally within a Learning Disabilities setting
  • Experience managing complex cases including safeguarding and risk
  • Experience working within multi-disciplinary teams and with partner agencies
  • Strong knowledge of the Care Act 2014 Mental Capacity Act 2005 and Human Rights Act
  • Strong assessment analytical and decision-making skills
  • Excellent communication and report-writing ability
  • Ability to manage workload and competing priorities
  • Competent IT skills for case recording and data management
  • Ability to work independently and as part of a team
  • Full UK driving licence and access to a vehicle (essential due to Somersets rural nature)
