Job description
CQC Supported Living Coordinator Earn £27,500 per annum and great benefits including Health Cash Plan Temporary (12 months contract), full time (37.5 hpw) Newcastle Upon Tyne We cant offer CoS for this role Home, a place where you belong Step into a role that makes a real difference every day, youll lead the way in creating safe, caring and responsive support for people with diverse needs. Youll make a big difference each day as you help our customers achieve their hopes and aspirations. What youll do Carrying out risk assessments, support planning, goal setting and regular reviews. Creating support plans with your customers and coordinating our team working to the plans. Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able. Supporting customers with their personal care and medication needs outlined in their support plan Support to attend appointments, take part in hobbies and interests or attend college or work Why join us? You have Level 3 Health and Social Care Certificate, or willing to work towards Passion to support our customers to live their best life, working collaboratively with an eye for detail. Experience of creating person-centred support plans and supporting colleagues to follow them. Experience in coordinating and assessing customer referrals. That may mean adapting your start or finishing times on occasion. 24/7 Rota pattern - Day shift and sleep shifts. Youll need an Enhanced DBS check, we pay for it!
