Job description
NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included. The role: To provide community-based tier 3/4 NHS specialist mental health assessment and intervention based on the principles of equality, dignity and respect and which takes into account spiritual and cultural diversity. Within the out-patient setting, care is delivered by multi-disciplinary teams in collaboration with partner agencies. It is based on the best available evidence for practice and is planned and delivered in a manner which promotes personal choice. The clinical team works collaboratively with service users, carers, families and their representatives to ensure that care needs are identified and met. All children/young people being cared for have a Case Coordinator who is tasked with working closely with their identified patients/carers to develop a participative, therapeutic relationship and deliver high quality specialist care in an environment where patients feel valued and where confidentiality is respected. Duration, Location, and Working Pattern: Permanent, Full time Base location - Templeton Business Centre Shift Pattern - Monday – Friday 9-5 Key responsibilities: Clinical and Care Coordination * Act as Named Nurse/Practitioner for a complex mental‑health caseload, providing care coordination within a multidisciplinary team. * Complete holistic assessments, including risk assessment, and plan, implement, and evaluate specialist care programmes. * Deliver nursing care in the least restrictive and most therapeutic manner. Patient Wellbeing and Safety * Identify and respond to mental, physical, and health‑promotion needs, supporting overall wellbeing and relapse prevention. * Recognise and address issues related to vulnerability, protection, domestic abuse, gender, and self‑harm. * Build therapeutic relationships that respect equality, dignity, cultural and spiritual needs, using interpreter or partner services when required. Clinical Interventions and Treatment * Provide evidence‑based psychological therapies within competency limits. * Administer and monitor medication and treatments, including managing side effects. * Work in partnership with patients and carers to deliver person‑centred, values‑based care. Professional Standards and Governance * Ensure all practice complies with NMC Code and relevant legislation (Mental Health Act, AWI, ASP, CYPA). * Contribute to strong clinical governance, including audit, evidence‑based practice, learning culture, and risk‑reduction strategies. * Maintain high standards of record keeping. Supervision, Leadership and Development * Participate in regular supervision, appraisal, and professional development planning. * Provide caseload supervision, clinical supervision, and reflective‑practice facilitation for others. * Support and assess learners and students, contributing to a high‑quality learning environment. * Monitor and support the performance and development of junior staff and healthcare support workers. * Provide nurse leadership and undertake management duties when required. Partnership Working and Service Contribution * Work with statutory and non‑statutory agencies across hospital and community settings to support coordinated care. * Contribute to service development, operational guidance, and quality improvement within the CMHT. * Participate in working groups and multidisciplinary projects to promote best practice and service redesign. Knowledge, training, qualifications and/or experience required to do the job: Essential * Registered Mental Health Nurse or RNLD (Level 1 NMC registration) * Educated to minimum degree level or equivalent * Completion of an evidence‑based psychological therapies/psychological approaches programme * Specialist knowledge of mental health nursing, including physical healthcare * Specialist knowledge of risk assessment and factors affecting mental health, illness, wellness and recovery * Evidence of active and ongoing CPD * Evidence of providing Clinical Supervision/reflective practice as a supervisor * Specialist knowledge of rights, values‑based practice and legislative frameworks * Evidence of digital literacy, including use of information systems, email and basic applications * Experience of working in a community environment * Demonstrated application of specialist skills and behaviours, including: o Organisation and applied leadership o Self‑direction and motivation o Enhanced written and verbal communication (negotiation, influencing, conflict resolution) o Decision‑making * Ability to work flexibly, prioritise, delegate appropriately and work without direct supervision * Ability to form therapeutic professional relationships and work effectively within a wider team * Working knowledge and evidence of applying the NMC Code * Ability to undertake multiple clinical contacts in home/near‑home settings, including driving Desirable * Working towards postgraduate qualification or equivalent * Completed recognised mentorship preparation programme / registered mentor * Evidence of CMHN Career Development Framework Level 6 (Senior Practitioner) across: o Clinical practice o Facilitation of learning o Applied leadership o Research and development * Experience of care coordination * Experience of line management responsibilities What we offer: We offer a wide range of supportive policies designed to enhance your employee journey, including a comprehensive Employee Assistance Programme, Cycle to Work Scheme, bursary scheme and extensive learning and development opportunities.
