Overview Seeking Speech-Language Pathologists in the Home Health setting for Fort Worth and the surrounding areas! Job Description The Speech-Language Pathologist is responsible for comprehensive assessments in the patient\s home, including the completion of skilled documentation through Homecare Homebase. * Evaluate patients and perform appropriate therapy interventions * Develop a comprehensive plan of care and coordinate multi-disciplinary care * Perform clinical documentation and visit notes * Continued communication with physician, care team members, patient and family * Comfortable working in all aspects of the home health setting * Implement and maintain SLP protocols * Manage patient treatment and outcomes directly Requirements * Current Texas Speech-Language Pathology License * One year of SLP experience * Experience with Homecare Homebase is helpful, but not required * Previous Home Health experience is helpful, but not required * Current Texas driver\s license * CPR certification Job Type * Full-Time Medical Specialty * Home Health Schedule * Monday to Friday License/Certification * BLS Certification (Required) * Speech Therapy License (Required) Work Location * Multiple locations/On the road EMR Used * Homecare Homebase Benefits * 401(k) * 401(k) matching * Dental insurance * Flexible schedule * Health insurance * Paid time off * Referral program * Travel reimbursement * Vision insurance Experience * Speech-Language Pathology: 1 year (Required) J-18808-Ljbffr
