The role:

An exciting opportunity has arisen for an experienced Registered Nurse, holding a firstlevel adult nursing degree, to join our team as a Band 5 Community Staff Nurse.

The District Nursing Service delivers skilled nursing interventions across a broad range of clinical needs, providing compassionate, personcentred care that places individualsand those important to themat the heart of all decisionmaking.

In this role, the post holder will practise as an independent and autonomous practitioner, supported by a strong foundation of theoretical knowledge and clinical expertise. A commitment to ongoing professional development is essential.

Duration, Location, and Working Pattern:

Permanent, Full-time

Shift Pattern: 36hrs 4 shifts over 7 days

Base location - Vale Centre for Health & Care

Key responsibilities:

  • Assess, plan, and implement programmes of care in partnership with patients, families, and carers.
  • Temporarily manage the caseload during short periods of colleague absence, seeking senior support when needed.
  • Use clinical and environmental risk assessment tools to identify risks and implement appropriate interventions.
  • Report and participate in reviewing incidents to prevent recurrence.
  • Support patients, families, and carers to achieve optimal health and independence.
  • Prioritise and manage workload, delegating tasks appropriately to ensure patient needs are met.
  • Contribute to maintaining a highquality 24hour nursing service.
  • Provide patient care within Treatment Room or Health Centre settings as required.
  • Apply flexible and innovative problemsolving approaches when barriers exist in patients homes or circumstances.
  • Participate in clinical audit and research to improve practice and care quality.
  • Implement evidencebased nursing interventions aligned with national standards and guidelines.
  • Adhere to relevant local, regional, and national policies, procedures, and guidelines.
  • Provide clinical leadership and act as a positive role model for junior staff.
  • Manage own daily workload as delegated by senior nursing staff.
  • Take part in practice support systems, supervision, PDP processes, staff meetings, and required training.
  • Respond effectively to emergency situations using basic life support skills.
  • Ensure practice aligns with NHSGGC policies, NMC Code (2015), and relevant legislation.
  • Maintain accurate clinical records and care plans in line with NMC and data protection requirements.
  • Supervise and support preregistration students and visiting multidisciplinary staff.
  • Participate in placement audits and implement changes with education partners.
  • Support delivery of preceptorship programmes for newly qualified staff.
  • Assist with induction and supervision of new and junior team members.
  • Maintain responsibility for own professional development and support the development of colleagues.
  • Establish effective communication and collaborative working with primary, secondary, tertiary, statutory, and voluntary services.
  • Participate in discharge planning and development of coordinated care packages.
  • Understand determinants of ill health and contribute to health needs assessments and health improvement activities.

Knowledge, training, qualifications and/or experience required to do the job:

Essential

  • Applicants MUST hold a full driving licence with access to a car
  • Part 1 NMC Registration, (Adult Nursing)
  • Ability to work as part of a team
  • Ability to work on own initiative

Desirable

  • Previous experience within District Nursing is desirable but not essential
  • Leg ulcer management
  • Palliative Care
