Responsibilities

  • Provide expert and professional social work services to adults/children, their families and carers by undertaking outcome focused assessments to identify eligible needs, developing outcome focused care and support plans which maximise independence and potential. Monitor and evaluate care and support plans and check the standards of care are in line with local authority processes and legal requirements.
  • Establish, develop and maintain professional relationships through direct casework and supervision, with the aim that identified needs are met. Work with other agencies to facilitate effective service delivery and ensure best outcomes for individuals.
  • Ensure assessments, planning, implementation and reviewing of caseload and decisions comply with legislation, timescales, policies and procedures. Maintain accurate, timely and evidence-based records and meeting review timescales.
  • Assess and report on adult/child at risk in accordance with safeguarding procedures, including investigations and strategy discussions with the Police where applicable.
  • In collaboration with the Team Manager/Assistant Team Manager, act as independent chair of continuing health care, care and support meetings, adult safeguarding meetings, school attendance meetings and their review conferences, ensuring records are accurate and actions are identified.
  • Promote participation of individuals needing care and support, their families and carers in assessment, planning, implementation and reviewing; offer advocacy when appropriate.
  • Use resources to deploy and maximise financial and material support to individuals and ensure value for money in care packages.
  • Monitor and check standards of practice to support quality assurance, including auditing and sampling of files, and contributing to service improvements.

Context Statement

Provide social work services to adults/adults at risk and children/children at risk, their families and carers by undertaking outcome focused assessments to identify eligible needs and develop outcome focused plans that facilitate care and support. Work within the Council and external organisations as applicable, following professional protocols, policies and procedures and legislation.

  • Support the team manager to undertake quality assurance and champion improvement of processes and services within the team/wider service.
  • Independently manage conflict and caseload decisions within a framework of regular supervision.
  • Advocate for individuals and challenge discrimination or disadvantage arising from practice; represent individuals and the council in court proceedings when required.
  • Collaborate with colleagues, council services, agencies and external organisations to facilitate effective service delivery and best outcomes.
  • Engage in what matters conversations and undertake specialist assessments to ensure best interest decisions are made for all casework.
  • Pursue specialist recognition in two key areas (e.g., Practice Educator, AMHP, BIA, DoLs/AMCP) and complete CPD to support ongoing re-registration and role requirements.
  • Be encouraged to undertake CPD that enables further relevant social work qualifications within defined timescales.
  • Operate and deliver services using modern, technology-enabled approaches and diverse communication channels.

Person Specification

Qualifications

  • Social Work Degree or equivalent qualification
  • Masters in Social Work or equivalent qualification
  • Consolidation Award / Post Qualifying Award
  • Two Specialist Awards (or equivalent) with commitment to obtain within an agreed timeframe

Register and maintain registration as a Social Worker with Social Care Wales

Specialist Knowledge

  • Current and up-to-date knowledge of relevant social care legislation for adults and children’s services/education
  • Local Government legislation
  • Evidenced knowledge and senior practice experience as a social worker

Practical and Intellectual Skills

  • Ability to write and present high-standard reports
  • Ability to organise and prioritise work
  • Proven skills in assessment and safeguarding
  • Ability to represent the service in court, chair related meetings and advocate on behalf of stakeholders
  • Understanding of the role and organisation of social services
  • Ability to develop key relationships with other teams, services, agencies and organisations
  • Ability to work independently using own initiative
  • Ability to communicate in Welsh
